Should I Be Concerned About The Uterine Fibroids At The Age Of 55?

I am a 55 year old African American woman. I had my last menstrual period in February 2012. Last week, I had a full week of a menstrual cycle again. It was because of the profuse bleding that I went to the emergency room and then the next day visited my GYN doctor. I was then sent for an ultrasound...here are the results:
Transabdominal ultra sound: The bladder appears normal with a volume of 68cc. Uterus measures approximately 12.9x6.6x6.9 cm. the right ovary appears normal. The left ovary is not satisfactory seen. the endometrial exho strips is not well-defined transabdominally.
Transvaginal pelvic ultrasound: the endometrial echo complex measures 8mm in double wall thickness. There are four fibroids demonstrated (Then it gives the size of each fibroid). The left ovary measures approximately 2.7x1.8x1.8 cm and appears normal. the right ovary measures approximately 3x 1.5x2.4cm and appears normal. There is no free fluid seen.
Impression: Uterine fibroids.Top normal thickness endometrial stripe for age. The endometrial stripe is sonographically homogenous. Norma appearing ovaries for age.

Is there anything wrong with me? Am I like to have cancer?

Hi,

According to your ultrasound report:
- You are having bulky uterus, possibly due to uterine fibroids
- Ovaries are normal size to your age
- Endometrium is of normal thickness but for your age it is at upper normal limit and homogenous

Usually median menopause age in African American women is 49 years. But some women may have menstrual cycles up to 55 years also.

You didn't mention the sizes of fibroids,
- If the fibroids are of smaller size like less than 5 cms, you can take anti fibrinolytic drugs to control the bleeding, with your doctor's advice. If bleeding is still there, you can go for progesterone supplements.
- You may need to undergo endometrial biopsy once to rule out endometrial causes of bleeding.

Usually the size of the fibroids will decrease after menopause. So, usually surgery may not be needed. But if the fibroids are of bigger size like more than 5 cms, and if the excessive bleeding continues in next cycles, not responding to medicines you may need surgery.

According to your history and ultrasound, there are no specific features confirming malignancy, but better to be careful and get evaluated.

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Sree Gouri S. R., OBGYN
