Itching In Perineal Area With Dry, Scaly And Peeling Skin. No Help From Any Creams. What Could This Indicate?

hello: I have been trying to treat an itch situation in my perineal area for several months now. I do have several hemorrhoids that contribute to the problem, however the itch (rash) encompasses the entire area between my but cheeks. It gets really raw and then after a few days (and applying every treatment cream under the sun) the skin will dry, get very scaly and peel off like paper. The itch is persistent. I have used otc hydro-cortisone, creams for fungal infections (also otc). The only thing that gives SOME relief is vaginal itching cream, but that is not adressing the problem. I did go to see my family doc, who referred me to a GI specialist who was only interested in scheduling a colonoscopy. He sent me off without really addressing the problem! I am desperate. I can not sleep, go out in public, or just go about regular daily activity. WHat could this be????

Welcome to HCM.

I am suspecting fungal infection as that area is more prone for fungal infections. Another reason could be pin worm infection which also causes severe perianal itching which increases during the night. You can get a perianal swab test done for microscopy and fungal culture which will rule out both possible infections. You need to get a prescription for appropriate antifungal or antihelminthic based on the test results. Keep the area clean and dry. Avoid steroid creams. You need to get in touch with a dermatologist for examination of the rash.
