Hello and welcome to HCM,
Before answering your questions, I would like to give you the normal values of semen analysis:
1. Volume: 2ml per ejaculate
2. Sperm concentration: 2 million per ml
3. Motility: At Least 50% sperms should have rapid progressive forward motion
4. Morphology: At Least 50% sperms should have normal morphology
5. Pus cells, red blood cells, abnormal cells should not be present

Your report shows semen volume of 2.5 ml which is normal, rare or few actively motile sperms and majority of sperms have abnormal morphology.
This report is showing sub-normal values.

Since, all the parameters are not mentioned, it is not possible to comment on the report.
You need to post all the parameters for an opinion.

Thanks and take care
