Is It Necessary To Have Radiation After Total Hysterectomy Due To Endometrial Adenocarcimoma?
Good morning. I had a total hysterectomy as a result of a endometrial adenocarcimoma, FIGO 2 with focal clear cell features, depth of invasion was <50%. All lymph nodes negative. The suggestion is to have three radiation treatments into my vagina. I wonder why it is necessary and I am considering not doing the radiation. What else could be suggested for follow up long term, if the risk of clear cell is microscopic?

I can understand your concern. You do not want radio as you feel the tumor is not spread so why the radio. Clear cell carcinoma is a very aggressive cancer and has a high potential of spread. The FIGO stage is not always 100% accurate. Sometimes it is not possible to sample all the nodes, or sections examined from nodes may not show tumor while other pieces that were not examined may have tumor. Even though these possibilities are not very common, but if clear cell is suspected, radiotherapy is advised considering the aggressive nature of this particular malignancy.
So you will be taking a small risk if radiotherapy is avoided. Some missed foci may get cleared by radio. So discuss this once again with your doctor and then choose. Hope this was helpful.
