Intermittently High Fever, Deep Cough. What May Be Wrong?

My wife has had an intermittent high fever for several days now. Two nights ago it got up to around 103, but then seemed to break. Yesterday she was fine. Last night she started up a pretty bad deep cough . Now her fever is back (above 101). Any thoughts? We re in a small town in Ukraine, so good, western-style medical help isn t available.
