How To Overcome Habit Of Masturbation And Depressive Thoughts?

I am 32 years old, and have had a problem with chronic masturbation since piuberty. My background conditions are Multiple Sclerosis with chronic fatigue, and depression. I am addicted to pornography, and often use it as a form of escape, or to take my mind off of depressing thoughts. I have tried to stop many times, but always make an excuse to do it again. What treatment options are available, and how do I approach a pyschologist, or Psychiatris, to ask for help in dealing with masturbation? Over coming the embarasment of admiting I have a problem has been troublesome too.
