If any one of your sex partner is HIV reactive, then you are at risk for HIV.
As you knew well about the sexual activity of your sister or grandmma ?, you can decide accordingly. If any one of your sex partner has multiple sex partners then also you are at risk. Ejecting outside the vagina does not matter.

HIV only spreads from a HIV positive person to another. It is recommended you do not have multiple sexual partners. Using condom will definitely lower the chances of getting any STIs including HIV. It doesnt matter if you ejaculate outside or inside a vagina, the chances if getting a STI remain the same.
