Have Hypertension And Bipolar Disorder. Swollen Limb After Accident. Suggested Approach?

Hello,
I am a 46-year-old female with well-controlled hypertension and bipolar disorder. I suffered a Blunt trauma in the inner part of my right upper arm 8 days ago in a car accident. The inner part of the whole limb turned purple with major swelling near the inside of the elbow. This hematoma has resolved two-thirds with application of ice yet the localized swelling remains and is now turning slightly yellow- it feels like a 2½ by 1½ inches mass of gelatin wedged within two firm cords. I feel well otherwise- no weekness, no peripheral vascular anomaly, no fever, no nausea, no distal oedema. I only have localized hypoaesthesia in the swollen area which is neither red, tender or warm. What approach do you suggest? Thank you! Christine

Hi, thanks for writing to HCM.

The initial management of the Hematoma is as follows -
* Rest to the injured part and don't involve in activities which puts additional strain on the injured tissues.
* Ice packing should be applied for about 20 minutes once in every 2 hours.
* Use elastic compression bandage around the swollen area
* Elevation of the limb
* Analgesic & anti inflammatory medications

Generally, the hematoma will resolve spontaneously within 3 weeks (maximum) without any issues. Hypoaesthesia is transient and will recover over a period of time.

If you are a diabetic person, the chances of a hematoma getting infected is high. Otherwise everything will settle down with time.
