Diabetes Mellitus. Been Having Hypoglycemic And Orthostatic Hypotension Episodes. On Medtronic Paradigm Pump. Suggest

My husband has diabetes mellitus, is on the Medtronic Paradigm pump, and for the past 6-7 months has been having hypoglycemic episodes and orthostatic hypotension episodes. He checks his blood glucose level throughout the night. His basal rate has been set to accommodate the nocturnal phenomenon. He has been seeing his endocrinologist of longstanding for regular checkups. Your input would be greatly appreciated. Thank you.

Hello,
While it is true that hypoglycemic episodes can sometimes be fatal,it is also true that is is very common among diabetics.It is one the most dreaded complications of diabetes treatment.Paradigm which was designed by medtronic was to address this issue.Not only does it send alarm signal when blood sugar is below certain level,but also suspends the dose of insulin.So,with this the hypoglycemic complications can be controlled and also sugar may be monitored very close to normal,which is necessary to avoid secondary complications.Hence it is not necessary to be worried about the intermittent episodes.
Orthostatic hypotension is a different condition in most cases and requires further investigation.Dehydration due to uncontrolled sugars may be one connecting cause between the two.Other endocrine cause need to be ruled out.Autonomic tests need to be done to rule out autonomic abnormalities.

Hi, Thanks for using HCM.

Since the basal rate is required through out the day and no need to adjust it. But based on his glucose level and the food he eats the intermittent injections should be adjusted. If more it can cause hypoglycemia. For orthostatic hypotension, he need to do regular exercises every day to maintain the tone of his muscles and ask him to drink plenty of water. If you can put his sugar level values and BP values at different condition for further discussion. Hope I answered your question. Consult your doctor for further examination and management.
