Hello,
As per your description, it seems likely that you may have had an abortion. The sac like substance could be the products of conception. Your symptoms of pelvic congestion, missed period, pinkish spotting at about the time of the expected period which could mean an implantation bleeding are all suggestive of pregnancy. To know if you are/were pregnant, you can get an estimation of the serum beta-hCG titres, which can read positive for 3-4 weeks even after an abortion. A trans-vaginal/pelvic sonogram will help to know if there are any products of conception in the uterus. Hope this helps.
