Question: my son is 2 yrs 9 months old and weighs about 12 kg. he is having a fever from yesterday evening. i gave him 4 ml paracetamol ( 250mg/5ml syrup). fever comes back within 6 hours. he doesn't have any nose congestion or cough or sneezing. he had complained about a pain in his legs a few times when the fever came back. today evening, even one hour after giving Paracetamol the fever hasn't reduced. there is sweat in his back. but still the temperature on his forehead is 40°C. I'm planning to give him meftal p syrup( mefenamic acid 50 mg/5ml) which was given by our paediatrician in the past to give if the fever is not reduced after giving Paracetamol. is it ok to switch the medicine? what should be the dosage? What should we do if the fever doesn't improve? should we wait for one more day or should we go to hospital tomorrow?

Fever of few days without any localizing signs could as well a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all. Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.
Paracetamol can be given in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.
Meftal-P contains Mefenamic acid which is a NSAID. It carries the risk of gastritis and renal problems in the long run. Please do not use Meftal-P.

yesterday morning, the fever appeared only after 8 hours of previous Paracetamol dose. but later the fever began to return after 6 hours. Today also, the fever has returned after around six hours. is it normal? or should we be worried? he was active yesterday and played with his elder sister. still has no cough or sneezing.

after 3 days, my kid's fever is gone. now, he has red rashes all over his face, palms and behind his ears. There are a few rashes on his back as well. Is it normal after fever? should we apply anything on the rashes?

It is very much normal and understandable.
Unless and until he develops the danger symptoms which I have discussed in my first interaction the Rash need not be worried about.
You may apply any topical emollient lotions over the Rash... like Atogla.
If you are worried please upload the images of the rash and I will see and get back.
Don't worry as long as he is not having any danger symptom I suggested please.
