Question: hello doctor, My 15 months old baby has been having cold since last 4 days. He also had his 15 months vaccination 2 days back. The cold was mild when it started but now one nostril of my baby is stuffed badly. He is also having fever which has gone till 101.2. He is very restless and crying a lot. He also has his one eye swollen now. And there is almost a continuous white mucus discharge from the nostril of same side. He is otherwise a very active baby but because of all these he is either crying or just want to sleep in my arms. The fever does get subside for 2-3hours after taking the medicine but is back again afterwards. We also had done Nebulization today in hospital but still there is no improvement. I am attaching the prescription from his doctor. Baby's weight is 10.3 kgs and i have completely weaned him off breastmilk. Even after taking medicines baby is very restless and appears to be in pain. Please suggest. Thanks

Hello,

If there was no swelling of the eye and if your son was only having some fever, then we need not have worried.

It seems like your son has developed an acute infection in the nose and eye. And since he is crying constantly, he needs to be examined by a paediatrician to rule out anything serious.

I do not think it is worrisome but in children, it is extremely important to be careful.

Please do consult his paediatrician and follow the advice.

For the fever and nose block you can give Dolocold Syrup at 15 mg per KG of body weight.

If his weight is 10 Kgs then he will need 150 mg of the medicine.

Please visit the paediatrician and take appropriate treatment.

Medical History: 23th Aug - Common cold and cough 25th Aug - 15 months vaccination prescribed Ascoril and Allegra for cold and cough by doctor 26th Aug - 101 degrees fever started in night 27th August - Mild Swelling in the right eye started with continuous discharge of white colored mucus from Right nostril ,Nebulization done in hospital 28th Aug- Extreme swelling in right eye.Consulted with Eye specialist as well. Recommended oral antibiotics and cold/cough as the reason for coughing. Blood test and chest x ray done. Since CRP was 46, doctor startef IV and stopped oral antibiotics 29th Aug- 3 sessions of IV over now. Swelling in the eye has reduced a little but fever is still coming after 5-6 hours medicine. Fever is between 100 and 101. Hello Doctor, Above is the medical history for my 15 months son. I have also attached the test reports and precriptions. Please suggest if we are on the right course of treatment. He becomes very lethargic and sleepy the moment fever starts setting in. His meals have also reduced to 1/3 of what he eats usually. On top of that he now has the prick done on his leg for IV , so, we cant let him walk also after his meals. Hence, he vomits sometimes. Thanks, Neha

Thank you very much for writing back to me with all the details, it is of immense help in understanding the ongoing issue, I appreciate it greatly.

I do not see any reports attached, the one doctor's note attached earlier is very small so I can not read anything that is written.

That said, I am relieved to know that the doctors have admitted him and have started IV antibiotics. This is what I would have done too.

CRP of 46 does indicate severe infection, and hence the IV antibiotics are absolutely a must.

Paracetamol can be given at a dose of 15 mg per Kg body weight every 6 hours once.

Antibiotics need at least 3 days to show appreciable response, however, since you are noticing that the eye swelling is reducing, I think the infection is getting under control and eventually it will clear off in 1 week.

Please do not get your child discharged from the hospital early, the IV antibiotics should be given for a minimum of 7 to 10 days in such severe infections.

Please do not panic, the treatment is going as it should.

Please ask your paediatrician to rule out meningitis by doing CSF examination.

If meningitis is suspected, then antibiotics need to be given for longer periods and at double dose.

You do not need to let him walk after meals, you should put him on your shoulder and tap on the back slowly till he burps, that will reduce the vomiting issue.

It is quite difficult to eat like normal when we are sick, it is okay if he can not eat as before, he will eat well again as soon as he gets well. Please do not worry about this right now too much.

Please keep me posted about how things go, I will try and guide you to the best of my ability.
