Will Deep Brain Stimulation Help In Curing Brain Stroke?

Question: My dad suffered a stroke around 15 months back and MRI report says "BILATERAL THALAMI INFARCTS".
We have tried ayurveda treatment for 3 months and physiotheraphy for almost 1 year,but noticed no improvement in his health. We coducted Stem cell therapy around 5 months back in Mumbai. and also we found that he has brain atrophy. He is better now and can observe around 2% of improvement.

He is 60 years old.

His current status:
1. He is bedridden, unable to walk and walks only with the help on shoulders.
2. He speaks very slowly and most of the time, we are unable to understand what he is saying.
3. He answers our queries, when we ask him, but never tries to start a conversation and he will be always sitting silent in the chair.
4. He forgets everything like, what he had for breakfast and he doesn't remember most of his life time events and he just knows his close family members name.

Is there any brain surgery is possible to improve his health conditions? I heard about deep brain stimulation, does it help him? Please let me know all the available treatments for his case.

Thanks for contacting Ayurveda with your health concern and following points are worth mentioning from Management point of view:

1. Since possible cause of stroke as suggested by your history:
. no hypertension, on ecosprin [aspirin] suggests ISCHEMIA to be the likely cause thus one must have following insight:
i. because ischemia causes lack of blood flow to the brain thus results in symptoms of inability to move, inability to understand and inability to see. [present in your history]
ii. Once a stroke is completed, some brain tissue is dead and its function cannot be restored. [as evident by history: brain atrophy]. Do consult with your Doctor which area is/was damaged and what functions it probably served!

PS. In the future, perfusion CT, functional MRI, and tractography using diffusion imaging in stroke patients may provide a better understanding of the role of the corticothalamic relationship in behavioral changes associated with thalamic stroke.

2. any history of alcohol consumption, high cholesterol level, Smoking, previous cardiac disease [since hypertension/diabetes are excluded]

3. forgetting anything and sitting silent: means higher functions could be hampered with frontal lobe involvement.

4. Since the case is diagnosed and he is in recovery stage, [nervous system takes time to heal] and probably bed ridden, thus prior to initiate/suggest medicines, please keep following things in mind:
. change the position every 1-2 hours to avoid bed sores.
. feeding should be done preferably in sitting position.
. give protein supplements, avoid foods that may cause diarrhea.
. give enema on alternate days to evacuate the bowels.
. sponge the patient everyday with a wet towel followed by dry towel, followed by talcum powder.
. Physiotherapy is advised to prevent joint contracture and edema of the limb, and to promote recovery of strength.
PS. Passive movements of the limb also prevents venous stasis and pulmonary embolism
. check every 2-3 days for bed sores over sacrum,back,greater trochanter and ankles. If bed sores is developed, dressing & padding.
. avoid soakage of urine or stools, as bedsores develop very quickly in such cases.
. Chest physiotherapy should be started to promote coughing and prevent pneumonia.
. Rehabilitation: speech and occupational therapy should be tried.

5. The recovery process requires pancha-karma therapies and rehabilitation measures because standard ayurvedic treatments are always individually formulated.
. Mix 100 gram of covered kidney bean powder and dried ginger in 2 cup milk and give it to drink daily for 1 month.
. Prasarni or XXXXXXX tail for local massage
. Prescription drugs available for paralysis are: ekangvir ras, vrihat vaat chintamani ras,vaat kulantak ras,vishtinduk vati, yograj-mahayograj-kaishore gugullu, maharasnadi-rasnasaptak kwath, dashmool-bala-ashwagandha-draksha: arisht, Rheumalya, R-Compound, Rheumanil,gunja tail.

i. Deep brain stimulation (DBS) has been reported to be successful in relieving the motor symptoms and motor fluctuations in the more advanced stages of the disease, however, data on the effects of DBS on speech performance are inconsistent, thus your attending doctor/neurologist is in better position to guide you.
PS. since implantation of DBS itself is associated with risk of stroke/hemorrhage along with memory problem thus it is best to consult a Neurologist for further opinion and management.

ii. since muscles are mainly affected due to the stroke, so in order to prevent unused atrophy "abhyang chikitsa' is given.
