Orthostatic hypertension, what is it? What causes it?

Brief Answer:
I would explain as follows:

Detailed Answer:
Hello,

I passed carefully through your question and would explain that your symptoms could be related to orthostatic intolerance,.

This is a cardiovascular disorder, which leads to a drop in blood pressure values during standing up position from sitting or laying position.

This is a disorder affecting the autonomous nervous system, which can be related to an autoimmune disease.

A Head Up Tilt test is the golden standard tests to rule in/ out this disorder.

It is usually triggered by an infection or a metabolic disorder ( chronic anemia, thyroid gland dysfunction, diabetes, etc. ).

Regarding the increased sleepiness, you should know that insulinoma and low blood sugar can trigger it.

I recommend discussing with your attending physician on performing a Head Up Tilt test.

Hope you will find this answer helpful!

I remain at your disposal for any further questions whenever you need!

Kind regards,

You didn't read my question well. My BP went up 20 points when I went from laying to standing. I never been so dizzy in my life too. New problem too. I was diagnosed with sleep apnea a few years ago. I was having some health problems at the time. The symptoms went away and I quit doing the CPAP. Last year I had bypass surgery. One symptom I had was waking up imonths an I was on the verge of a heart attack. Now I woke up and had several incidents of SOB. I breath normal and then all of a sudden my body would take a deep beat, like apnea breathing but I was awake. See history. I am also going to a Neurologist. because of bad dizziness and h/a. It appears to be a nerve problem. I was tested by laying down flat and then sitting up. My BP went up 20 points and I was so dizzy I had to be helped to stand.

Hello again!

I am sorry for the misunderstanding!

Please note that it is quite physiological to have a higher blood pressure during standing up position compared to sitting position.

But the dizziness in such case could be related to two possible problems:

- an inner ear disorder
- thyroid hormone dysfunction.

Another possible cause to consider would be vertebro-basilary insufficiency.

Coming to this point, I would recommend consulting first with an ENT specialist and performing labyrinthine tests in order to exclude an inner ear disorder.

It is also important to check your thyroid hormone levels for thyroid dysfunction and performing a Doppler ultrasound of the carotid and vertebral arteries to investigate for possible narrowing.

Considering the fact that you have suffered from sleep apnea, which is a chronic disorder, I would recommend performing a new polysomnogram to check if there is any need for CPAP again.

Hope you will find this answer helpful!

If you have any other questions, please don't hesitate to ask me again!

Best regards,

Dr.Iliri

I do understand but have been told that for my BP to be 20 points higher on standing is outside the normal range.

Brief Answer:
I would explain as follows:

Detailed Answer:

Yes, this is an exaggerated reaction.

For this reason, I recommend checking your thyroid hormone levels and your inner ear, because vertigo, which may be triggered by position changes, can lead to uncontrolled high blood pressure, which corresponds to standing up position.

Now I am afraid I may have to go back on a CPAP. I had the diagnose of Sleep Apnea but I haven't had a problem for awhile 2 years or more. I have 2 episodes in last two weeks, I first wake up SOB. It last a few min. The 2 incident have been in the afternoon after a nap. Today I was weak and it took several min. to feel I could get up. I took an ASA extra strength and some hot pepper, thanks to my husband. I was working very hard in the garden moving soil and getting ready to plant a fall garden. I had a like incident just before my bypass surgery but I was at 7,000 ft and had just woke up from a night sleep. Now what?

Hello again!

Thank you for the additional information!

Although you have been symptom free during all this time, an exacerbation of sleep apnea or coronary artery disease, can not be excluded.

Coming to this point, I would recommend performing a new polysomnogram and a new exercise cardiac stress test.

Hope you will find this answer helpful!

Wishing good health,
