What Is The Cause And Treatment Of Balanopothitis?
Question: Hi Doc, I dont have a very retractable foreskin as such. But couple of days back, all of a sudden i noticed that the penile hole almost got closed. Also there is a bit of irritation on the tip. What can be the cause? I am married. All of a sudden, how can the foreskin close itself?

I had no problems couple of days back. All of a sudden while urinating, i noticed the flow of urine was not proper. So i tried to pull the skin back a little and found that the skin had almost covered the penis hole by almost 85-90%. There's just a little opening left for urine to pass. The skin is not retracting beyond that point. Is it an infection? Because i think the veins around the penis skin tip seems to be a bit swollen and there's a constant irritation (very mild). I have one follow up question left, so i hope this detail is enough Doc. :) do you recommend a cream or any oral antibiotic that i can take?

The cause of your symptoms is likely to be due to Balanopothitis (inflammation of glans and foreskin). The thickened secretions could have formed a hardened scab on the urinary opening leading to blockage of opening.
I would suggest the following
1. Hot Seitz Bath: To a tub of lukewarm water add 2 spoons of dettol. Sit in it for 15-20 minutes. Pull back the foreskin as much as possible. Clean it gently. Do not try to remove th scab off the opening, doing so will result in severe stenosis
Do it twice a day. Take refreshing bath after it.
2. After bath , dry the area thoroughly. Pull back the foreskin and apply equal mixture of Soframycin and Betnovate Ointment. Apply it thrice a day
3. Clean the Penis and foreskin thoroughly after passing urine every time

Thanks a lot Doc for the detailed answer. Just let me know is this a serious problem or a widely prevailing thing which is common and nothing to worry for me. I am really stressed due to this. Also, how much time is expected for my recovery if i follow the above procedure as told by you. Is there a need for me to take any anti-biotics along with a cream such as ultravex?

Thanks for all the replies earlier. So i went to a so called big shot Urologist in XXXXXXX and as anyone can guess, the straight answer was i need circumcision and there's no way in the world i can be cured without it. He didn't even had the manners to talk or diagnose anything with a caring attitude. Anyhow, what i am looking for is a routine i need to follow each day and the timeline i can be cured within.
I am sorry about not uploading a pic yet but i'll try in my next follow up as i am generally in the office.
What i can make out with my internet research is that i all of a sudden have a classical case of pinhole phimosis with reasons unknown. I have no itch, irritation (almost gone now), discharge, odour or anything terrible. What i am left with is the foreskin covering most of the urethral opening and isn't retracting.
I have started with stretching exercises but if you can guide me the correct procedure and whether to do it with or without creams.
Also, which creams do i use and for how long. As mentioned by you earlier, a combination of Betnovate/Soframycin. So for how long do i persist with this combination and how many times a day. I think Betnovate is for relaxing the skin right while Soframycin is for the infection if any?
Biggest question, how come at this age of 32 all of a sudden this can happen to the foreskin?
I am against being circumcised for many reasons and i wont opt for it. Please help me out with hopefully some encouraging example so that i can let out all the depression i am going through. Is it safe to have sex as of now?

It is perfectly okay if you do not want to opt for circumcision. I also do not recommend circumcision straight awayThe ointments suggested by me will be helpful. Betnovate is Steroid, it helps in preventing further adhesion formation between glans and prepuce. Majority of cases do improve with it. Soframycin is antibiotic ointment. Take a sterile plastic container and empty both the ointment in itt. You can apply the mixture twice a day for 3 weeks. You can see the improvement in a week time. Slow gentle retraction of prepuce starting 1 week after initiation of application of ointment is all that is required. Forceful retraction of prepuce will create raw area and further predispose to adhesions. Betnovate will also make the prepuce soft and supple.
The problem still persists simple prepucial separation is all that is required. Providing a clinical photograph will be helpful to know the exact status of problem.
