Question: I have low back pain and a hx of 4 lumbar surgeries w fusion...I had bilateral lumbar RFA at 3 levels plus SI joint block last year which helped a lot. ...I was finally weaned off of long term opiate therapy...About a month after weaning, low back pain returned.. plus rt Flank pain...MRI lumbar repeated and showed large kidney stones too large to pass. Stone rt renal pelvis upper pole 1.7cmx 1.1 cm. With mild hydronephrosis...left kidney stone 9mm in left lower pole... Was scheduled for O PATIENT lithotripsy but didn't have $700 upfront to pay...sent to County hospital in XXXXXXX Tx.. Urologist can't do surgical removal of stone til 1/2015...How will I know if Hydronephrosis is worstening?? And what point would M.D. consider doing Emergency Nephrolithectomy??

Brief Answer:
J stent may be placed to relieve obstruction.

Detailed Answer:
Good Day and thank you for being with Healthcare Magic!

Hydronephrosis means there is obstruction in your right kidney possibly because of the 1.7 cm stone in your right kidney. If you have intractable
pain, fever, and increase in your serum creatinine then your Urologist would need to do an emergent decompression of your right kidney by putting in a double j stent to relieve the obstruction and your renal pain.

For your left kidney since there is no obstruction then I would suggest medical treatment to possibly decrease stone size, Potassium citrate tablets are usually given to dissolve renal stones.
