Brief Answer: Panic disorder has to be treated Detailed Answer: Hello Thanks for the add on information. Your symptoms seem to be of Panic disorder with agoraphobia and are severe enough to warrant treatment. Abilify, trihexyphenidyl will not relieve anxiety much and xanax (alprazolam) will only be a symptomatic treatment. You will need treatment with proper anti-depressants such as venlafaxine, escitalopram, mirtazapine, paroxetine are good drugs in this series. This is a disorder of anxiety and occurs cause of imbalance of certain neurotransmitters in the brain. especially serotonin, nor adrenaline, and dopamine to some extent. Medicines are the best options to take care of these symptoms. Paroxetine is the the most popular medicine and my drug of choice. I generally prescribe my patients 12.5 mg per day at night which takes care of anxiety symptoms. Also certain medicines like propranolol and clonazepam can be give as required when there is a panic episode or expected situation where panic attack can appear. Also there is an option of cognitive behavioural therapy (CBT). This therapy is taken by either a psychologist or a psychiatrist in which the therapist identifies impaired cognitions (Thoughts) and try to correct them. They are 15-20 min weekly session and there should be around 8-10 sessions to help you with your illness. Hope I am able to answer your concerns. If you have any further query, I would be glad to help you. If not, you may close the discussion and if possible you may rate the answer for me, so that I get a good feedback. Brief Answer: Zoloft should be good Detailed Answer: Hello Thanks for the followup question. Yes zoloft should be a good drugs for you. It will take care of your depression/anxiety. You need to continue your abilify, trihexyphenidyl and xanax for the management of schizophrenia. However the anxiety symptoms have to be treated along with the management of schizophrenia. Yes, you should be able to go to the school and start driving with zoloft. Its only with xanax that you have to be careful as the reflexes would be slow. Hope that helps regards Dr. Srikanth Reddy

Answered by Dr. Srikanth Reddy (32 minutes later)
Brief Answer: Paxil and zoloft are equally good Detailed Answer: Hello Thanks for the follow-up. Both paxil and zoloft are equally good. Yes it would be wise to get shifted to klonopin instead of xanax. Abilify if its working well, then it can be continued. If i were your treating Doctor, I would like to put you on Quetiapine - antipsycotic as you have propensity for movement disorder Paxil- for anxiety Clonazepam - for sedation and anxiety.
