What Does This Ultrasound Scan Report Indicate?

Question: Hello Doctor,

I was asked to go for a scan as I missed my period this month. I have regular periods for 5 days. 28 days cycle. So i was due on 21st this month. I did have unprotected sex with my hubby 2.5 weeks back. My pregnancy test came negative when i tried once on 27th morning. My doctor asked me to get checked for polycistic ovary. My scan report is as below.

"Urinary Bladder - Distended. No Calculi.
Utreus - Anteverted, normal in size. Endometrial and myometrium echoes are normal. Endometrium measures 11-12mm.

Ovaries - Both ovaries are bulky in size, shows multiple follicles which are peripherally arranged with increased central stromal echogenicity. A dominant follicle in the right ovary measuring 1,9X1.5cms with hemmorrhage.
Left ovary : 4.1X3.0X2.1 cms (13.3cc)
Free fluid in the POD with floating echoes (h/o WDPV)

Impression : Bilateral polycystic ovarian pattern
Findings suggestive of PID."

Kindly let me know if I have to do another pregnancy test now or do ihave PCOS?

Hello,
Thanks for the query to hcm,
The repot and pictures confirms that you have PCOD with multiple subcentrimetric follicles lined along the border of the ovaries with central echogenicity and an endometrium of 12mm with a hemorrhagic cyst in the right ovary. + left over has volume more than 10cc, which is indicative of pcos again.
Please get a hormonal profile done with serum testosterone, 17OHP, and DHEAS levels as well which shall give an estimate about the androgenic content.
If you have acne , hirsutism( increased bodily hair growth) and acanthosis nigricans ( darkening of necklace area) then clinical hyperandrogegism shall be confirmed, which would further reiterate the fact that you are a case of POLYCYSTIC OVARIAN SYNDROME .
And this is primarily the reason for your delayed menses as well.
