What Does This Ultrasound Report Indicate?

Question: Dear Doctor,

My 4 years daughter Ultrasound report stated an elongated mixed echogenic lesion (35.5 x 14.2 mm) with multiple cystic space seen in right lobe of liver, show no obvious increased vascularity within. There is no evidence of any internal flow in lesion. Rest of liver parenchyma show homogenous echo pattern. Intrahepatic biliary radicles are normal. Portal vein is normal in calibre.
Please suggest what does it signify and follow up advice.

Thanks for writing in to us.

The first ultrasound scan report mentions a 43 x 27 mm area in the right lobe of liver which is likely an injury due to trauma.

The second ultrasound scan report shows a 35 x 14 mm area and there is a decrease in the size.

From the above we conclude that there was an area of injury in the right lobe of liver which is healing. This is a good sign and there is going to be likely complete recovery in few weeks.

In such cases nothing is to be done and the liver will get back to normal after some weeks. Usually no active treatment is required since there is decrease in the size but you can get an ultrasound scan done after 15 days to know the status of healing.

Dear Sir,
Thanks for your time.
But actually in first ultrasound done after 24 HOURS of incident size is 35 x 14 ; and in second ultrasound (done after 48 HOURS) it reported as 43 x 27 mm.
Please comment.
My second quarry is, what does echogenic lesion (35.5 x 14.2 mm) with multiple cystic space mean ?
Can it affect normal liver function ?

Dear Sir,
Thanks for writing back with an update.

There appears to be an injury to the liver and the size of the clot has mildly increased between 24 to 48 hours.

The description of echogenic lesion with multiple cystic spaces refers to the area of likely injury in the liver.

If the patient general condition is good and there is no internal bleeding then it is not a serious problem.

In the reports there is no mention of and features of active internal bleeding due to injury and I hope the pulse and blood pressure are normal.

The liver is a big organ and this injury is unlikely to affect the function.

You can choose to do a review ultrasound scan as recommended or after 3 to 7 days from last scan if your daughter is stable. It is important that your daughter is kept under medical observation as required by the treating doctor.
