What Does This Ultrasound Report Indicate?
Question: Hi, this is for my wife who is 37 weeks pregnant, she started having pain above her stomach for a few weeks but it got worse the past few days and the doctors finally did some blood work, results are attached.
her background....
35 year old, weighs 150 lbs now (113 before pregnancy), experienced some hair loss
takes prenatals everyday
takes synthroid everyday her thyroid levels elevated after the first month
possible mthfr deficiency
about the baby...only 5 lbs at 34 weeks ultrasound, very low AC at 3%
should I be concerned for my wife or baby?

I really can't see the results attached here. Please reattach them. Also any pain during pregnancy and over the uterus should be evaluated for foetal well being and onset of labour and also to be investigated for any antepartum hemorrhage. Please attach the USG report.

The reports are more or less fine with borderline raise alkaline phosphatase and raised WBCs which can be due to pregnancy stress as well, but my concern was the pain and hence the ultrasound pelvis was a must to rule out any abrupt placentae or APH or whether the foetus was not under distress or if it was a preterm labour depending on the per vaginal examination for condition of internal os.
The blood report is not going to help much in this case. Please review with the ultrasound and the clinical reports your wife.
To me it a preterm labour unless proven otherwise.

hi how is it preterm labor if she is at 37 weeks? will an ultrasound of the liver and gallbladder show anything or does she need ultrasound of pelvis too?
Also, I attached my wife's 34 week detailed ultrasound, does this show if she has placental abruption? Pages 1 and 2 are from 20 weeks and 3 and 4 are from 34-35 weeks.
I added another report also from UPenn from 35 weeks that we did for her as well

Hello,
These reports are fairly normal and shows early growth retardation in mid april scan. But my concern remained the present sudden onset pain abdomen which called for an urgent ultrasound to establish the cause of the pain, hence i suggested that you should seek obsterician's consultation in ER. if the pain has subsided by now and there has been no bleeding or any uterine contractions and the baby is moving normally in situ then its fine and she can be observed but if the pain is still present then she would need to be observed for next 48 hrs under medical supervision.
As i saw she has a history of false labour pains in mid april as well and early growth retardation had been noticed , so i feel its more likely premature onset of labour rather than an abruptio. There is a possibility that she may be in latent labour. Is she getting uterine contractions where the pain and contraction comes, stays for 25-30 seconds and then goes away? If this is so then she is in labour.
Any pain during this time, should be thoroughly investigated.

thank you, but is the baby healthy? what kind of ultrasound does she need for the abruption? is the liver and gallbladder bladder ultrasound enough? say she does have abrupt ion what nextm

The ultrasound has to be abdominal and pelvic combined and if its an abrupt then an emergency c-section is the next step else baby would be compromised.
Hence an urgent usg is recommended.
