Question: Hello Doctor,
Pulmonologist I visited today told me that I'm allergic to aspergillus and has recommended me to continue with my current medicines.
I'm attaching my reports. Looking forward for your kind advise.

I can understand your concern.
I have gone through the reports you have attached.
Your chest x ray is normal. No need to worry for this.
Aspergillus fungus causes chronic lung infection. It causes central bronchiectesis, but your x ray is not suggestive of this. Actually HRCT is more informative for this.
We need to get done total serum IgE and Aspergillus specific IgE fir the diagnosis of allergy to Aspergillus.
So please let me know
1. Have you ever undergone HRCT?
2. Have you ever undergone serum IgE level?

Thank you for the response Doctor.
1. I have not undergone HRCT.
2. I have attached my IGE report.
I am very worried Doctor as my health is not well since couple of weeks.
My symptoms are 1. Shortness in breath 2. Spasm feeling in my back & sides 3. Chest tightness. 4. Sometimes tiredeness in shoulders & chest.
I don't have phlegm accumulation, no wheezing, no crackling sounds while breathing, no cough.
From 2 days, i have a feeling of fine dust ( fine gunny threads inhaled when a gunny bag is dusted ) in my throat and wind pipe and soon after that feeling i develop spasms and chest tightness.
From past two weeks, i have been taking many medicines (symbicort 2+2 times, spiriva 18 mcg one time a day and montelukast. Today, doctor has prescribed me prednisolone 30mg for 6 days.
Somehow i am not fully convinced to start oral corticosteroids. I have not found a good pulmonolgist in place i reside, thus looking for online help from a specialist.
Doctor here ruled out COPD or any condition other than Asthma without hearing me out. I just wanted to convey him that one of the Doctors has only mentioned about COPD changes after going through my X-ray and history.
Looking forward for your kind advise as its now badly affecting me psychologically.

I can understand your concern.
I have gone through IgE report.
It is 1022. It is considered as high. IgE indicates possibility of allergic disease.
Please let me know whether PFT (Pulmonary Function Test) is done or not.
PFT is must for the diagnosis of asthma or COPD.
It will also differentiate between these two conditions. It also tells about severity of the disease.
And you need specific IgE levels for further evaluation of allergies.

Dear Dr XXXXXXX
Please find attached all my recent reports.
My Doctor here is not recommending PFT. I have requested him many times though.
All he has to say is when my PEFR remains 570 to 600 l/min all times, it doesnt require PFT.
I am taking all prescribed medicines but still i am experiencing shortness in breath, back & sides pain (spasm feeling ) and tiredeness. Spasm feeling gradually reduces after taking medicines.
I would be visiting my hometown next week, first thing i will get a PFT done and will share the report.
I would be looking forward for your kind reply.

Dear Doctor,
Thank you very much for your kind guidance.
I would like to clear one last query.
I have consulted GP today due to cold, running nose, sneezing, cough ( changing from dry to with slight mucus ) since 02 days.
He has diagnosed it as allergic bronchitis and has prescribed gemifloxacin mesylate 320 mg antibiotics for 05 days along with ambroxyl hydrochloride syrup and loratadine 10 mg tablets.
I would be glad if you could guide me on interaction of these drugs with my current medication - symbicort & spiriva inhalers.
Would you recommend me to go ahead with this course.

Possibility of viral upper respiratory tract infection (URTI) is more likely in your case.
No need to worry for interactions. But better to take azithromycin instead of gemifloxacin. Azithromycin is good for atypical coverage .

Dear Doctor,
Thank you very much for your response.
I have started azithromycin 500mg antibiotics course fir 3 days from today along with mucolytic syrup and loratadine tab. I am continuing my current medications too.
1. I have not heard of adult pulmonary vaccination.
2. I have not undergone pneumococcal and influenza vaccines.
I am coming down to XXXXXXX next week, will enquire more about mentioned vaccinations.
