Brief Answer:
Chronic sinusitis

Detailed Answer:
Hi
Welcome to healthcare magic and thanks for putting up the query
From the prescription and symptoms it appears to be sinusitis. You should get a x-ray paranasal sinuses done and send me the pictures.
The treatment mentioned in the prescription is absolutely right. Further management will depend on the reports.
You can get an endoscopic examination of the nose done by an ENT SPECIALIST.
There is probably nasal septum deviation and mucosal hypertrophy.
Follow up with the required information so that I can suggest you better.
Any further questions are always welcome.
Take care
Thanks and regards
