What Does The Following Blood Test Report Indicate?

Question: Hello doctor,
my wife is 36th week of pregenancy, please note the below details for your information,

Date: 17-08-14
glucose challagne test with 75g glucose
Blood sugar - 126

GTT, Date: 10-10-2014
Fasting blood sugar - 114
fasting urine sugar - nil
blood sugar after 1 hour - 210
urine sugar after 1 hr - NIL
blood sugar after 2 her - 210
blood sugar after 3 hr - 113

FBS PPBS result date 11-10-2014
FBS - 100
Fasting sugar - nil
PPBS - 138
PP urine sugar - Nil

Our doctor insisting that, my wife sugar level is high and she needs insulin to control. once they start the insulin they will diliver by c - section.
But me and my wife worrying about cessarion.
my wife already in diat, and she avoide all sweets, rice and other suger items.
wheather she really needs insulin? till date her condition is totally normal, baby movement is good, she working fine, till today she is attending office.
in the second opinion, another doctor told, she is completly normal with her sugar report, but she need strict diat to control PPBS 138 to 110.
can any one help, is she normal? is she really require insulin?

I understand your concern..

As per the reports provided only 1 value of GTT is high,that is the second value. Other wise the rest of value is normal.The repeat FBS and PPBS are within the normal range..

She is prone to have diabetes in future. Right now she needs to have good diet control. Avoid sweets. Increase the vegetables in the food. Intake of carbohydrate rich diet should be reduced. Intake of chappathi is preffered to rice. Avoid starvation. Avoid oily and fried food.

Repeat FBS and PPBS weekly till delivery. FBS should be less than 110, PPBS less than 140. If there is uncontolled sugar levels, she may require insulin..

Is this her first pregnancy? Is there any history of stillbirth or abortions?

Get a scan done to see for fetal growth, estimated fetal weight and blood flow to the fetus. If estimated fetal weight is less than 3.5 kg with adequate liquor and good blood flow, normal delivery can be tried.. If you have a recently done scan report, kindly send it.

Keep a check on baby movements..

Thanks for writing..

By this scan the growth is corresponding.. There is a mild pelviectasis of one kidney which needs a follow up to see whether the size is increasing..

Get a repeat scan by 1 week to see for fetal growth, weight and degree of pelviectasis. If everything is normal, you can try for a normal delivery with close monitoring..
