What Does The Following CT Scan Report Indicate?

Question: I have been got to know on 25th XXXXXXX that I have a stone in ureter when I got a very pain full attack,
doctor gave me injection like voveran , bascopan , and drotin then after three injection I got relief,
then via CT Scan he told me that I have a
RIGHT DISTAL URETERIC CALCULAS WITH PROXIMAL HYDROURETERONEPHROSIS ,
There is a 2.9 X 3.8 mm (AP X CC) calculas in the RIGHT DISTAL URETERIC CALCULAS WITH PROXIMAL HYDROURETERONEPHROSIS
then he prescribed me medicine like
TAB ZIFI 200 BD FIVE DAYS
TAB DROTIN BD THREE DAYS
TAB TAMFLO O.4 MG OD FIVE DAYS
Then again on 27th XXXXXXX I got a pain attack then again I got injected with vovaran , bascopan , tramadol and emeset, then I got a relief
Then again in I got a pain attack yesterday night at 2 then I got injected with Vovaran and Bascopan
then I got relief
today I got my doctor changed and got my KFT TEST DONE FOR WHICH I HAVE ATTACHED THE REPORT
and I have got my ultrasound I the report were :-
THERE IS A MILD HYDRONEPHROSIS AND MILD HYDROURETETER 2 X 3 MM CALCULUS AT THE JUNCTION OF MIDDLE OF 1/3rd AND LOWER 1/3rd URETER RIGHT SIDE
today Doctor has prescribed me some medicine :-
Ketorol - DT 2 TABLETS (SOS)
voveran injection im and atropine injection 1/2 iv and 1/2 im (sos)
Bascopan Tab three times in a day
tamflo 0.4 at bed time
wysolne 30mg with breakfast for 5 days

My query is :-
1. Have a I been prescribed with the right dose and medication and advise??
2. My report is abnormal so have I got any risk for my attached KFT Report ??
3. After every alternate day I have to take injection for pain is it right ??
4. IS MY KIDNEY IS FINE OR NOT, IF NOT THEN WHAT IS THE CURRENT SITUION RIGHT NOW
5. FROM LAST 72 HRS I HAVE CONTUINES PAIN ONLY WITH INJECTION IAM GETTING RELEIF

Hi,

The treatment prescribed is correct. However the pain may disappear after a few days even if the stone doesn't come out. Medicine may not be able to remove stones in all cases. So even after completing the present course of medications, even if there is no pain do repeat an ultrasound and check the status of the stones/hydro nephrosis . if ultrasound is not clear get a CT-KUB. If stones still present review with a urologist for removal of stones.

The slightly elevated creatinine is likely to be due to a combination of blockage + use of painkillers like diclofenac, which can increase the creatinine.

1. so what should I do for correcting my creatinine
2. according to my reports is my kidney is fine or not and if not please explain
3. how will my kidney hydro nephrosis will get better
4. stone in right is effecting left kidney also ?

Avoid painkiillers

Detailed Answer:
Your reports and history suggest that creatinine might be raised due to recent painkille use. So re doing your creatinine levels in about 6-7 days will help.

in the meanwhile. stop any painkillers..

usually a stone on one side will not affect the function of the other kidney, .

hydro nephrosis will disappear on either side after treatment of the obstructing. sometimes if the hydronephrosis was large in size, the hydronephrosis may persist for longer duration. although the blockage would have been treated

Kindly let me know the effect of the following medicines differently :-
Ketorol - DT 2 TABLETS (SOS)
Bascopan Tab three times in a day
tamflo 0.4 at bed time
wysolne 30mg with breakfast for 5 days (specially let me know for this medicine in detail)
also let I would like to tell you that apart from bascopan tablet iam not taking any other pain killer, so iam taking that thrice a day, so is that ok ?? as you told me to stop taking any pain killer !!!

Hi,

Thanks for writing back.

Ketolog dt contains ketorolac : painkiller - NSAID- can also cause creatinine to rise
buscopan: antispasmodic- to relax the ureters .
tamflo: to soften the prostatic urethra - eases the passage of stone out of the urethra
wysolone: glucocorticoid: as an antiinflamatory, reduces the oedema of urethral mucosa, making it easier for stones to pass down .

Hello Doctor,
1.Today again I got a pain attack at 11 AM in morning, so I had 2 keterol DT Together, again it is paining right now, so can I repeat 2 more tablet again of
2. keterol DT, I mean in 12 hrs how may tablets I can have it,
3. what can be the cause of pain ????
4. also can I repeat my ULTRASOUND Tommorow, as I did it 5 days back so I hope there is no side effect of getting ultrasound in such a smaller period of time,
5. Iam again having problem of urinating, it is paining in the tip of my penis,???

Pain is likely due to stones

Detailed Answer:
The pain is due to the fact the stones are still there. repeated ultrasounds do not have any side effect.
However a CT - KUB (kidney, ureter, bladder) will give much more information than an ultrasound.
Pain at the tip of penis suggest some stone passing through the bladder or entering your urethra.
Get yourself reviewed by a urologist if symptoms persist, rather than taking repeated painkillers.
