Question: HAve a pain in back left part of skull-sharp-but not always there-goes down through the neck

I can understand your concern. Your pain is sharp shooting, intermittent and radiates upto neck. Such type of pain is mostly due to "nerve". It can be

: Occipital neuralgia - Greater occipital nerve (C2) which supplies back of head, neck can sometime trapped and cause severe radiating pain. The pain increases intermittently, sharp, current like.

Diagnosis is by clinical symptoms and area of pain. It does not involve front part of skull.

: Cervical spondylosis - Cervical (neck) disc prolapse and nerve compression can cause radiation of pain from neck till occipital area, vertigo, radiation of pain in shoulder/hand, numbness and tingling in extremities.

It can be diagnosed by clinical symptoms, examination and MRI cervical spine.

: Cervical region mass compression cord/nerves - Mass like neurofibroma/schawannoma can cause compression of cord and nerves. They can present like above mentioned symptoms.

Diagnosis is confirmed by MRI cervical spine (plain+contrast).


: Less commonly neck muscle injury - Neck muscle like trapezius, occipitalis which are attached to skull and neck both, injury can cause pain in back of skull which is present in neck too. Usually pain is dull, severe, increase on stretching of muscle, neck movement.

It can be diagnosed by examination, feeling tenderness over neck. There will be no tingling, numbness, radiation, but only dull, severe pain.

Thats a relief
I was worried about stroke etc at my age (62)
I did start working out and did a pretty heavy 2 weight lifting sessions although I didn't work that area-but possibly from back or shoulder exercises which I did push hard

In ischemic stroke, pain usually does not occur. In hemorrhagic shock, severe headache with loss of consciousness, vomiting occur.

I advise you to consult doctor. It is because the pain is in neck and the differential diagnosis includes cervical disc and neuralgia.

These conditions need to be ruled out. If you will ignore it, it can cause problem in future. So to avoid any big problems in future, you must show it doctor and at least get yourself examined.
