Question: A three year old boy has red warm area under his toes area with a visible red line dividing the portion of his front part of the foot from the mid portion and then the same red warm area under his heal area with the warm and red skin.

Before running into final conclusions, I'd like to know if your child is also having rash any where else in his body (face, hands, mouth, etc.)? When did the rash appear (within a week, month, or more)? Has he been suffering of eczema earlier? Is the affected skin scaly too? To my opinion, it might be Sweaty Rock Syndrome (juvenile plantar dermatosis) from your description and the photo. However, psoriasis and hand-mouth-food disease should be ruled out first. Sorry. He does have rash on his face at time as well. He has had this foot disturbance for over a month at least. The affected skin area is scaly, dryed.

Brief Answer: Advised to run lab test for fungal infection... Detailed Answer: Hi, Then, most probably is Sweaty Foot Syndrome. However, I suggest to run the microscopical examination of skin scrapping with 10% KOH to exclude fungal infection. Meanwhile, the following home remedies can be helpful. The main scope is avoid his feet from becoming wet: - make him wear cotton socks - change his socks once they get sweaty/wet - try to use gentle cleansers (no soap) - make him wear sandals or open shoes ; no rubber or plastic soles - wash his feet with lukewarm water and gentle cleanser - dry them well - apply Vaseline (or olive oil) at night, while during the day, might apply dimethicone cream. Once again, I insist to do the above-mentioned exam first to exclude fungal infection as might need other specific treatment (anti-fungal). Hope it was of help!
