Question: Sir my daughter 3 months is having pus cells 150-200/hpf and albumin - present +++. Kindly suggest reason and remedy. Please.

Thanks for the query

I have seen the attached reports.There are two reports done on 28/3/15 and 29/3/15. Both of the reports are showing significant pus cells and albumin in the urine.
Presence of pus cells in urine suggests that there is urinary tract infection.This may be of upper urinary tract involving kidney or lower without any involvement of kidney. Now the presence of albumin(3+) suggests that there is involvement of kidney.As per report I would like to rule out pyelonephritis.

As you have mentioned the culture of urine has been sent.All babies below one year of age with UTI needs evaluation by USG followed by DMSA(renal scan).
DMSA should be performed after one month of treatment for UTI.

The treatment of UTI is antibiotic as per the culture and sensitivity report.
Most of the babies of this age needs Intravenous antibiotic depending on the severity.As report is suggestive of UTI a presumptive antibiotic(cephalosporin group) treatment should be started as most of the time the causative bacteria shows sensitivity to this group.If the culture report differs from this presumptive treatment antibiotic should be changed accordingly.

So I would suggest you to consult your doctor for antibiotic treatment and screening USG.

Oral cefixime or iv ceftriaxone till the culture reports come

Hello

Welcome again

E. coli is the most common organism causing urinary infection. Both oral cefixime and intravenous ceftriaxone are effective in UTI.
As baby is of 3 months age, I would like to get a kidney function test(Serum Urea and creatinine) and CRP(for infection) done and to start IV ceftriaxone if feasible.
If intravenous therapy is not possible Cefixime is a good alternative.
