Question: PLEASE FORWARD MY QUERY TO AN INDIAN DOCTOR , PRFERRABLY MD(Medicine) R/s.. I have been suffering from Fever (Max tem 102.7F), cough, body ache and abdominal discomfort(heavy stomach though no losse motion) since 19.03.2014. I underwent certain pathological investigations on the said date and the findings are as under : CBC : Hb%-14.6, Wbc-6800, Rbc-4.80, Platelets- 1 lakh 13 thousand MP on Slide- Not Seen LFT : Sr Bilirubin-0.85, Direct bilirubin-0.35, Indirect billirubin-0.55 Widal : S. Typhi "O" - Agglutination upto 1:80 dilution S. typhi "H"- Agglutination upto 1:40 dilution S. Paratyphi "AH"- no Agglutination S. Paratyphi "BH"- No agglutination Urine Routine and deposit : Clear Further, I sought medical attention and am being treated with the following medicines : 1. Cefixime & Potassium clavulanate Tablets 2. Nimesulide & Paracetamol Tablets 3. Rabeprazole sodium and Domperidone Capsule 4. Tinindazole tablets 5.chlorpheniramine maleate wih codeine phospahte syrrup Sir, I would request you to advise me on following issues : 1. Does the investigation report says that I am infected with salmonella typhi and i am suffering from Typhoid..Or is this infection minor in nature 2. is the line of treatment and the medicines stated above are rightly advised and are meant to cure this . 3. would you suggest any other medicine or any investigation to be carried out now. 4. can i take pomegranate and milk and other fruits..what are the food items that are to be avoided

Brief Answer: Get Malarial Antigen test and Typhidot done Detailed Answer: Hi XXXX Thanks for posting your query on this forum. 1. ALL tests are NORMAL. 2. Widal's Test does not suggest S. typhi infection (Typhoid fever). As such, S. typhi O titers become positive only after 6-7 days from the onset of infection. Also, Widal's is a non-specific test. TYPHIDOT is a more reliable test to diagnose or rule out Typhoid. 3. I cannot comment on the prescribed treatment without getting the following additional information : - does the fever come up with chills and shaking and associated with headache, which lasts only till the fever is there? - How many hours does the fever last in a day? - is the cough dry or have associated phlegm? do you have symptoms of common cold? 4. Since you belong to an area which is endemic for malaria and viral fever, I suggest that you get Malarial Antigens test, which is more diagnostic than 'MP on slide'. 5. If you have intermittent fever (with chills and shivering) lasting for few hours in a day, associated with transient headache only during fever, it is malaria unless proved otherwise. In that case, I would therefore recommend that you take Tab FALCIGO 2 tablets twice a day on first day; followed by one tablet twice daily for next four days. 6. The other possibility is viral fever, which does not require any specific treatment except for Paracetamol 1000mg thrice a day + Levocetirizine 5mg twice daily + plenty of water daily and, bed rest. Consult your doctor and apprise him of my opinion. I am certain that he will agree with me; order the recommended tests, and- based on the reports- will arrive at a certain diagnosis and prescribe suitable treatment as advised. R/S ,thankyou for responding Fever : no chilling or extreme shivering before arrival of the fever..though i feel my shoulders, back , arm, and leg muscles aching intensely when fever is about to come..Sir what i feel is these muscular pains start normally after 7-8 hours of intake of medicine. After medicine intake they decrease gradually. though since yestreday fever is off yet these muscular pains are there. I have experienced No Headche while in before or after fever. cough : sir, initially it was dry but now the phlegm comes out while coughing Cold : sir i feel my nostrils blocked during the night though the nose is not running 1.) why does these muscles ache start after 7-8 hours of medicine intake and decrease gradually after i retake the dose. does this show that fever is still there. 2.) Dr. has prescribed DeeCee Forte tablet along with said medications..what is this for and should i take this any longer. 3.) your view on if the current medications and line of treatment is rightly advised 4.) what food and fruits should I take..

Brief Answer: It is most likely malaria Detailed Answer: OK. The features are suggestive of malaria more than viral fever. Muscle-pain recurs once the effect of Nimesulide + Paracetamol (pain killers) wanes off. There is no further requirement of DeeCee, Nimesulide and Cefexime. You should be all right with Falcigo and Paracetamol + cough syp as advised earlier. There is NO restriction of foods. Take NORMAL meal. Take care
