Brief Answer:
Strict diabetes control and physiotherapy

Detailed Answer:
Hi dear,
I have gone through your question and understand your concerns.
Most of your problems are due to the uncontrolled blood sugars.
Since your HbA1C levels are decreasing by this treatment, you should continue taking this treatment.
You should have a strict diet control and do regular walk and exercise.
For the muscular pain you are having, you should do physiotherapy, which will help you relieve the pain. You can also take an oral preparation containing gabapentin and methylcobalamine for this pain
For headache, you can take amitriptyline , after consultation with your treating physician.
Please upload the MRI films and reports so that I can review them and explain you in detail.
Hope you found the answer helpful.

Brief Answer:
Provide more clinical details

Detailed Answer:
Hello XXXXXXX
The mri findings are mostly due to diabetes and not directly related to headache.
You have to control your sugar and decrease your weight.
What is the character of headache, associated nausea, vomiting, light sensitivity or sound sensitivity, unilateral or bilateral , throbbing type or bursting type, sleep disturbances, anxiety, stress or any precipitating factors?
Any visual loss, seizures, loss of consciousness, stroke, cardiac illness?
Provide these details.
You can take amitriptyline with pain killers for headache.
Do get back with further details.
