What Does My Blood Test Report Indicate?

Question: my father has been just gone through a blood test and got the attached report .
Could you please have a look and do let me know if we need to worry about High alkaline phosphate and chloride as shown in report.
the blood samples have been taken after 10 hours of fasting (i.e at 7 a.m. in the morning)
