Brief Answer:
Biopsy is mandatory to confirm Cancer of Prostate

Detailed Answer:
hello
Thanks for query .
Raising PSA above level of 4 is always suspected to be due to Cancer Of Prostate unless proved otherwise by doing Ultrasound guided Prostate Biopsy which will reveal grade of cancer and whether it is confined to prostate or has spread outside the capsule of prostate .

The line of treatment is decided on the Gleason's score and grade of the cancer of Prostate as reported by Histopathologist ..

If biopsy turns out to be negative you have to monitor his PSA levels every two or three months .

If he has obstructive voiding symptoms due to enlarged prostate he needs to take treatment (Tomsulosin once daily )and has to be under care of qualified Urologist .

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.
