What Does High Bilirubin Level Indicate?

Question: Dr XXXXXXX XXXXXXX please.

Direct Bilirubin 8.56
Indirect Bilirubin 4.78
SGOT 100
SGPT 94
Gamma GT 175
Abdominal Screening:
Liver - Shows increased parenchymal echoes- measures 14.4 cms
Gall Bladder - Appears contracted

The patient is my neighbour and is very much worried about the high bilirubin reading. Is his condition very serious. He is getting admitted in a local nursing home tomorrow.

Is the line of treatment OK according to you? He is a moderate alcoholic for the last 25 years. What can we tell him to bring down his anxiety.

I evaluated your query thoroughly.

*Investigations prove Infective Hepatitis , non-surgical jaundice.

*As patient has been given oral medications for the same & not recovered well,
he requires hospitalisation for better early faster recovery.

*There is not any point of panic or worry as his condition is not serious, relax
he will be recovered absolutely fine, but will take certain time.

*During Hospitalisation, necessary supportive IV fluids, antibiotics, antiemetics,
Proton Pump Inhibitors, Multivitamin injections , other supportive care will help
him to stabilise faster along with immunity building.

*After 3-4 days of IV treatment , progress is monitored by the Gastrologist
& accordingly decision for switching to oral molecules.

*Total recovery time by enlarge is 3-4 weeks returning blood levels to normal.
@So don`t get panic even 1% , convey the patient, build up his moral
else he is not going to recover inspite of best treatments.
@ Till hospitalisation, convey to take plenty of liquids - preferably boiled water,
coconut water, home made juices, butter milk , light diet.
- avoid smoking / alcohol
- avoid all oily / spicy / nonveg stuffs
- maintain his BP , Blood sugar levels.

The patient has been admitted to a local nursing home that belongs to the doctor who was treating him. This patient went to a doctor primarily because of oedema in both feet and the doctor diagnosed infective hepatitis based on symptom and results of tests. Is oedema associated with infective hepatitis cases or is it due to other reasons? As soon as I get reply for this query, I will close this discussion. Thank you.

Respected XXXX, Hi good morning.
*Good patient has been hospitalised for the same as oral medications were not
going to recover him in concern with his reports.
*Oedema is associated with infective hepatitis cases ( due to inadequate protein
synthesis by the liver , altered lymphovenous drainage due to viremia even in
presence of normal S.Albumin & absence of Albumin in urine ) , simultaneously
one has to rule out Renal involvement by S.Cretinine.
*So far as alcohol is concerned you have to warn the patient if he takes even sips
of alcohol also there are all the chances for him to go into fulminant hepatitis
& even hepatic failure with cirrhosis even after complete recovery.
*Anxiety control is concerned( it`s only after full clinical recovery ) by means of
meditation exercises & YOGA.
