Since you are a smoker,it is important to rule out malignancy.

ANA may be increased in many conditions. It may be positive in normal patients, malignancy etc.It need clinical correlation. It is important to see other symptoms like joint pain, morning stiffness etc.

I saw a infectious doctor and she rules out infectious diseases, I do have an appointment with an ENT doctor tomorrow and f/u PFT and a CT scan in December. Both my parents and
grandfather all died from cancer, does that increase my chances of having cancer?

You are rightly advised for follow up CT scan.It is important to see increase in size of nodules.
Since infective etiology is ruled out,it is important to undergo proper workup for malignancy.CT guided biopsy can also be done.It can confirm nature of lesions.
