Brief Answer: Possibility of acid peptic disease Detailed Answer: Hi Welcome to Healthcare-Magic Greetings of the day I do understand your concern and apprehension. Let me again reassure you things can be taken care off. Upper abdominal pain at the age of 28 years in the presence of normal ultrasound suggest the possibility of acid peptic disease as the cause. It's very much reassuring to know that extensive investigation you have done are normal. Coming to the complaint of your loose stools with sense of incomplete evacuation of stools, it's likely to be due to Irritable bowel syndrome. As you have undergone rectal examination and found normal. I would suggest you to get Stool examination done to rule out possibility of worm/ protozoal infection as the cause of your symptoms. As you have noticed you saw blood in stools but the Physician could not find Hemorrhoids. Hemorrhoids is just one cause of bleeding . Stool examination along with test for occult blood and oval and cyst will definitely be helpful. Amoebic colitis could have such presentation. I would suggest you the following investigation 1. Stool examination: Stool microscopy for oval and cyst. Stool for occult blood. 2. Upper gastrointestinal endoscopy: Even if the symptoms persists after following the advise given by me. I would suggest you the following which will definitely help 1. Ensure good intake of water : 2 to 3 liter of water per day. 2. High fiber diet 3. Consume High fiber diet 4. Small quantity of frequent meal: eating 3 large meals a day will cause gastric distension and will aggravate pain. 5. Avoid spicy and oily food 6. Meditation will be helpful to reduce stress 7. Tablet Pantoprozole will be helpful for relieving pain. 8. Tablet Metronidazole 400 mg thrice a day for 7 days will be helpful for loose stools. Do get back to me if you need any further assistance, will be glad to assist you. Take care Best Regards Dr Deepak Kishore MBBS, MS, MCH Consultant Surgeon
