What Do The Following Test Reports Indicate?

Question: Dear Doctor,
My name is XXXX and age 42, working as a managerial staff in a manufacturing company. Normal morning get up at 5 Am and sleep at 10 PM .
The following preliminary test i have carried out once in three months for the past 4 years and the results are also indicated below
1. Weight - 64 to 66 Kgs
2. Height - 168CM
3. Blood pressure - 85/55 to 100/70
4. Fasting blood sugar - 68 to 75
5. postprandial blood sugar - 75 to 85.

No abnormality found. As a interest i have checked master health check up in near by hospital. The reports are enclosed for your reference.
From the report the followings are my questions,
1. FBS ans PBS are varying than regular check? Some time i felt too tired and low sugar reported by that time but MHC result indicated that sugar level is high? and interestingly PBS is less than FBS. Is it normal?? i should repeat the test some where???
2. Cholesterol level is quite high?? shell i continue the same trend or precaution??
3. X - Ray report enclosed indicated as normal.
4. Status of the ECG report?
5. Height - 168CM and weight 64.0 Kgs during mhc
6. Blood pressure - 90/70

Request your views and clarification of the above.
Apart from that please go through the entire report and suggest me what should i do further for fitness.

Would you clarify a few things for me?
# how did you monitor your FBS and PPBS before the MHC- by a glucometer or a laboratory test?

# after how many hours of fasting did you take the MHC blood test?

# any family history of diabetes, heart disease, high cholesterol or hypertension?

# are you on any medications or supplements?

# do you smoke or consume alcohol?

To answer your queries :
1) yes glucose levels can vary depending on the the type of sample (capillary or venous sample)
-yes it is possible for FBS to be higher than the PPBS
-please do an HBA1C and glucose tolerance test (with 75grams of glucose) at one, two and 3hour intervals.
This involves taking a fixed amount of glucose and seeing how your body responds to it.
Gives a good way to identify abnormal response (diabetic

2)your HDL needs to increase loose to 60 and LOL needs to reduce. Please follow diet modifications and exercise regularly to help with this.

Diet modifications :
-avoid refined grains and processed foods
-have whole grain and home made food
-include whole fresh fruits and vegetables.
-eat smaller but frequent meals (every 3hrs).i clue snacks like sprouts, nuts and seeds.
-avoid fried, oily foods, sugar, carbonated drinks
-limit caffeine to 2 small cups a day. Have plenty of water (about 3 litres a day)
-avoid refined oil. Use gingelly/coconut/canola oil.
Avoid ghee and limit sweet intake.

Exercise for at least half an hour a day. Fast walking will do. Take the stairs, when possible.
Quick short walks for 10 mins at a time also helps.

-check lipid profile after 3 months of these measures.

3)chest x-ray is reported as normal.

4)ECG is also normal.

5) your height and weight is fine. Body mass index is normal.

Just follow the above suggestions.
In addition to this, please have a treadmill stress test to check if your heart works fine under stress. Do all the tests in a reputed center.

Thank you very much for your detailed answer. The answer for your questions.

Would you clarify a few things for me?
# how did you monitor your FBS and PPBS before the MHC- by a glucometer or a laboratory test?
FBS and PPBS monitored by glucometer once in three months and in the lab once in year.

# after how many hours of fasting did you take the MHC blood test?
After 10 Hrs (Dinner at 9 PM and lab sample taken at 7.30 AM) Hope need 12 hrs???

# any family history of diabetes, heart disease, high cholesterol or hypertension?
In the no diabetes, heart disease, high cholesterol. Father having BP- Hyper tension (BP 140/85)

# are you on any medications or supplements?
No

# do you smoke or consume alcohol?
No Smoking and alcohol...but on rarely i will take beer (In case of joined with foreigners)

As you rightly pointed out i am taking lot of ghee sweets..

There is no exercise i am doing now, but alternate week i am claiming 1500 steeps (Up and down) in 40 mins.
walking only 30 mins (slow)
Fasting in Breakfast and lunch once in a week (Monday) and i prefer to eat rice with curd always.

Kindly advice further.

Thank you for the follow up and clarifications.

-There will be a difference between the values taken in a glucometer and of that taken in a lab.

-8 - 10 hrs of fasting is enough.

- most of the habits you mentioned need to be modified.
It is fine to eat curd rice. Include other food groups like protein and other cereala (ragi, millets) in your diet as well. Sticking to one type of doesn't provide you with all the vital nutrients you need.
Variety is good
-you need to go cold turkey on the ghee sweets. You may indulge once in 10 days,if that's not possible

-keep healthy snacks in hand (sprouts, nuts fruits) and eat before you get too hungry. Eat slow and chew food well for maximum benefits.

-exercise every day whenever you can. If you get 10 minutes do a fast walk or slow jog around your house or do skipping/sit ups. Doing this 3-4 times a day will make a huge difference.
If you enjoy swimming, that's great exercise without the stress on the joints.

-don't sit for more than a half hour at a stretch. The body deposits and metabolized energy differently (bad) when you do this. Get up and stretch or exercise while sitting, go for a quick round around your office. Any movement helps

I hope you will seriously consider the lifestyle and diet modifications. Medications should be the last resort as there are a lot of unwanted side effects.

-monitor your cholesterol and sugar as advised earlier and go for a yearly check up. At the next MHC, do a treadmill test as well.

Dear Doctor,
Thank you very much for your advise.
I am planning to go to next MHC at the end of December 2016 at Apollo KH hospital.
In that i will include Treadmill test with ECO and Ultra scan of abdominal with present MHC.
Apart from this any test to be included.?
My Bp level always found 85/55 to 90/60.. This is ok??? Normal 120/80
I have one more question.
I will get headache When i am sitting the back seat of car.. This is normal or should give attention for this.

1) I would suggest that you include thyroid stimulating hormone, free t3 and free t4 and vitamin D3 levels.

2) baseline blood pressure can vary between individuals. Technique of measuring also matter?
How do you measure your blood pressure?

If you have no giddiness or discomfort, or light headedness while changing position, then it is fine. Nothing to worry about.

3)about your headache:.
# does it occur everything you sit in the back seat?
# does it start right away or after you travel for a while?
# do you travel with windows open or or with air conditioning on?
# any nausea at the time?
# how does it resolve?
# any triggers that you noticed (lack of sleep or water, when you are hungry, when it is too XXXXXXX etc?
