What Do My Lab Test Reports Indicate?

Question: Hello Dr i am 40 year with 110 kg body weight, i am having a problem of severe gastric pain during night hour every 3 - 4 days apart, its a severe pain which come down after IV injection of Drotin and Pantop, i have undergone the all diagnosis my blood profile is normal except SGOT and SGPT slight higher HB1C 7.8, blood sugar on edge. previously never come to Diabetic test positive this is first time,
pain start middle of rib cage and get worse withing 30 minutes i have to rush to physcian and after pantop and drotin injection all normal withing 1 hour, i am on very low fat and spicy diet since 5 - 6 year now almost no fat and no white sugar diet, i take my dinner by 7 Pm with very light food, i am now depressed by abdominal pain which occurs any time in night every 4 - 5 days.

welcome to health care magic, I have gone through your question and reports you have attached with it very diligently and I will try to help you out in best possible way.

Ultrasound is suggestive of fatty liver grade III with gall bladder wall thickening which means your liver has infiltrated with lots of fats and progressively it's function will deteriorate if you don't take essential precautions and medicines, you also have liver enzymes SGOT and SGPT

Endoscopy is suggestive of pan gastritis which means inflammation of the stomach lining (mucosa)due to hyperacidity.

you have borderline diabetes with High HbA1c

Here I would like additional information for better assessment of the condition like
Do you smoke or consume liquor?
Have you recently gained your weight or you were overweight precisely
Have you been tested for thyroid disease like T3,T4 and TSH?
If possible upload me your blood sugar and lipid profile reports.

In my opinion you should follow these guidelines
1)Try to reduce your body weight by dietary restrictions and regular physical exercise which should be a long term goal.
2)You should take all the prescribed medicines regularly, you have given antiacidity medicines which will definitely help you, it may take time to act.
3)You should avoid fatty and spicy food, have lots of fresh fruits and vegetables along with diary product like curd, yogurt and buttermilk.

thank you sir for ypur reply i am a veterinarian with history of overweight since generations my brother is 125 kg, second thing my thyroid tests are normal,

You have fatty liver and pan gastritis. So I would advise you to try to reduce body weight as much you can do, which will definitely help you in long run.

Since your blood sugar is also borderline with high HbA1c, so I would insist you to get into shape by shedding of extra weight .

you can very easily reduce it by dietary restrictions and regular physical exercise

Meanwhile continue with antiacidity medicines which you are taking already,
You should avoid fatty fried and junk food
you should restrict your calorie to 1800 kcal per day.
Repeat your blood sugar test after 2 weeks or go for glucose tolerance test to confirm/rule out diabetes.

my prime and moat concern is why abdominal pain start i just want the reason and also weather i have to take iv injection every time, what precaution i should take to avoid pain, pain is really very serious and i am so depressed about this pain, please help me out of tbis situation

Inflammed mucosa of stomach is reason behind the pain

Detailed Answer:
Hello

Since the inflammation in the mucosa of stomach is there so whatever you eat, will definitely irritate it and may cause serious episode of pain

It will be there till the time this inflammation subsides

In my opinion you should take antacids like Gelusil or Digene before every meal and whenever you start feeling slight pain in abdomen.

You can take sucralfate syrup 10ml three times a day for the next 5 days, which will help in healing of mucosa.

If it doesn't help than you may require Pantoprazole IV infusion.

You should take liquid diet and easily digestible food like fresh fruits and yogurt or curd.

It might take another 5 to 7 days to heal it completely.
