Question: Hi I have a female friend with an ESR of 100, ferritin 23mcg/L, normal WCC and platelets, RCC 5.6x10(12), Hct 0.34, MCV 64fL, MCH 20pg, MCHC 315g/L, RDW 16.1% and Hb is 103g/L. She's been tired with a foggy head and gets heavy periods and pretty frequent colds. The lab results also commented on anisocytosis+, Microcytosis ++, Hypochromia ++, Eliptocytosis ++ and occasional fragmented cells. Is it possible she has Thalassaemia of some kind because she eats pretty healthy but very little red meat. The high ESR worries me. XXXXXXX thoughts?

I have gone through your question. I can understand your concern.
She has hypochromic microcytic anemia.

It can be due to iron deficiency, thalassemia minor or anemia due to some chronic disease. However her ferritine is in low range. So I shall name iron deficiency as the likely cause.

I would order for HB chromatography once to rule out thalassemia minor. If that comes normal then mostly it is due to iron deficiency and she needs iron supplements.

She should also search for any occult bleeding.

Her high ESR may be related to anemia. However there are other causes that raise ESR. Her treating physician's will know about it.
