Question: In a 3yo child, can a rash accompany a fever, small bumps on chest or face?

Thanks for asking on this platform

Fever with rash in this age is commonly due to some viral infection.Bacterial infection like scarlet fever can also cause fever with rash.Less commonly rickettsial infection may present with these symptoms.

The common viral infections are measles,varicella,dengue and coxaskie virus.

The character of rash and pattern of fever gives a clue to the diagnosis.

I would like to know

1)Does the fever and rash both appeared simultaneously or fever was followed by rashes ? Rashes appeared on which day of fever?(In varicella fever and rash both appear at a same time,in scarlet fever rashes appear on day two of fever while in measles on day 4)

2)On which part of the body rashes appeared first like face,chest or limbs?(measles rashes first appear on face while in varicella rashes first appear on trunk)

3)Is there any similar illness in the family?(Viral infection can spread through close contacts)

4)Is there any travel history?(may be there in rickettsial infection)

5)Are there any associated symptoms like cough,cold,redness of eyes,sore throat etc?(usually found in case of measles and scarlet fever)

Please provide details.I would request you to attach an image so that I better able to help you out.

In most of the viral infection no specific treatment is needed.In scarlet or rickettsial fever antibiotic would be needed.
Till then give him paracetamol for fever.Avoid using NSAIDS,as these may precipitate the rash if it is due to low platelet count.
You can apply calamine lotion on the affected area.

The bumps were very fine on her back, neck and torso but mom said it was sudden appeareing first on neck and chest. Wasn't there yesterday. No sore throat or red eyes but occasional coughing, no travel outside of XXXXXXX See attached photo

I have gone through the image.These small maculopapular rash with fever on the same day can be due to varicella(chicken pox). So I would like to rule out varicella first.

Chicken pox begins with itchy red rashes mainly in the area of scalp and neck area then finally involves arms and legs. These rashes progresses with vesicle(fluid filled small blister) formation.
The diagnosis is mainly by clinical examination and confirmed when vesicular rashes develop.

It can be due to other uncommon viral infection(it is difficult to diagnose in initial 24 hours unless the characteristic rashes develop).

I would suggest you to get her evaluated by a pediatrician for proper diagnosis and management accordingly.
