Question: this appeared out of nowhere on my sons lower back. no fever. it doesnt hurt and not raised

Try to clean it by rubbing with soap and water. If it is getting cleared, it is good and confirmed.

: Brown spots or liver spots or melasma - It is usually after 50 years of age and presents on face, hands and scalp, but it can present at any age and sun exposed area.

It is treated by treating low haemoglobin, avoiding prolonged sun exposure, correcting hormonal imbalance.

As it is painless, you can wait for some time. If it disappears, no need to do anything. If not, consult dermatologist for examination and confirmation of diagnosis.
