Brief Answer:
ok, several issues.

Detailed Answer:
First, a sedimentation rate and CBC and C reactive protein will show inflammation. Then where the inflammation is coming from is the question. We see this a lot with chronic viral infection almost always a not very mysterious virus (hepatitis C or HIV, rarely hepatitis B). Can be lupus or auto immune disease attacking somewhere.

This is relatively easy, you get the spot that is inflamed and you see under a microscope what is being inflamed. Seeing protein deposits or cells going into the area show different patterns of inflammation. The inflammation can also show particular areas which are being attacked. Vessels for example. This would cause vessel and circulation problems.
