What Causes Weakness And Muscle Twitches In Hands?

Question: Hi,

31 male, white.

Daily excerises no issue for months, dayjob is office work computer use.

3 weeks ago had the flu, stopped my daily excerise routine.

2 weeks felt better, but experiences some clumseyness in non-dominant hand, ie more likey too fumble small objects, perceived not having the dexterity i had previously.

Right hand shook if holding a full cup of coffee for longer than 10 seconds.

Unable to do lateral raises with dumbels without my wrist dropping. Weights do not feel heavier and was accomplishing 20 reps on both arms prior to flu.

Left hand unaffected.

Hand feels tight when clenching a fist, forearm slight muscluar tenderness to touch, but no pain experienced.

Muscle twitches are common across whole body for some time, but since recent onset with right hand i have been experiencing daily intermittent twitches localised to right arm.

Saw GP had an exam including hand strength, was refered to a Neuro and scheduled for an MRI of the head as well as an EMG. Was told to rule out possibility of MS.

Anxiety risen since GP appointment yesterday, almost dont mind the idea of MS compared to MND/ALS.

These symptoms/signs appear on the list for the onset of ALS, would appreciate your input, thanks.

I have read your query and understand your concerns.

ALS ( amiotrophic lateral sclerosis ) is very unlikely to be the condition that is causing your symptoms, this for two reasons:

1. ALS is a very rare condition in people below 40 years old.

2. There is a viral infection that triggered symptoms onset.

I personally have doubts about Multiple sclerosis ( MS ) too, because, despite the resemblance of symptoms, MS is a relapsing/remitting disease.

MRI and EMG will give the needed information to have the correct diagnosis, in my opinion, since the symptoms started suddenly, ADEM ( acute disseminated encephalomyelitis ) should be included in differential diagnosis too.

Apologies, but prehaps flu, might have been too strong a term.

Sore throat, run down, fatigued with evidence my immune system was working towards ridding a virus via bright yello mucus. But not very severe, no shakes and sweats, not bed ridden.

Also.

History of intermittent night cramps in my calves of medium to strong intensity.

Sensations of hot or warm water being spilled on my legs for a number of years, only occuring occasionally (maybe noticed once a week or two weeks).

Since writting this lying in a propped supine position on my sofa, using my phone, i have experienced a mild pins and needles on my effected arm, from my elbow down to my pinky an ring finger (carpel tunnel?)

Im resting on both eblows, but this only occurred on my bad arm. Any significance?

There is no need for a "strong" flu to cause ADEM too.

ADEM is a condition very similar with MS, and is an immune mediated inflamation of mielinated nerve fibers.

However, there is need for tests to achieve a correct diagnosis.

Previous symptoms that you describe may be related to several conditions including dietary deficiencies of vitamins and micro elements, stress and anxiety disorder and even MS.

The situation will be clear after the tests that are ordered.

Ok thanks for the follow up.

Just to be clear could tendonitis be the cause?

I was previously performing 20 lateral raises on each arm and i think i could have been using a weight that is too heavy.

I also carried a rather large radiator from my dads to my flat aslo up three flights of stairs. This was a real strain and i felt most of the strain on my forearms and shoulders (this was maybe one or two weeks prior to the virus).

I work in an office all day and lean on my effected arms elbow when using the mouse, i also paint small detailed models for a hobby, painting with my dominant left hand and holding them with my right had, which i occasionally lean on. I have only recently resumed this hobby and have spent considerable time each day dedicated to this activity hunched over at my desk.

Finally to summerise the symptoms that have failed to abate.

Stiff hand movements, but still functional.
Unable to bend wrist forwards without a lot of stiffness and restance.
Wrist drop when lifting 9kg weights (only when to the side ie lateral raise).

Perceived weakness, reduction in fine dexterity. But not entirely sure how much strength and dexterity i had in my right hand to begin with.

Fasciculations local to the base of my thumb and forearm, but also appearing on calvles, legs, back. Seemingly daily occursnce but not typically long lasting. Intermittend 4/5 times a day (noticed).

Tingling sensation with elbow bent over 90' angle (occasional)

Miniscule movements of digits, slighty waving from the position i intend them to be in.

The strength seems like it may have improved slightly, can now hold a full cup of tea for longer than 30-40 seconds without shaking. However stiffness seems to have increased.

Sensations of warm water on skin of legs (occasional, but more than previously occured).

Sorry if i havev repeated a lotnof information, bitni just wanted to paint the most accurate picture possible.

As for ADEM, I had a stiff neck and headache last night, as well as feeling generally run down.

However these have gone and i have no fever or any other signs of infection (to my knowledge).

I know this site isnt used for official diagnosis, but i would appreaciate a 2nd oppinion if possible, nothing personal, but two heads are better than one. Thanks.

Tendons or muscles strain usually cause pain and restrictions in movements, but don't cause neurologic symptoms such tingling, fasciculations, loss of strength.

So, I don't think it is a possible cause of your symptoms.

As per ADEM, since there were new neurologic symptoms after a viral infection, it is a possibility that may be included in differential diagnosis.

It is okay to have a second opinion.

Hello again,

I just wanted to add that i seemed to have lost quite a bit of weight over the last two weeks. Around 8lbs.

My next appointnent is not until February! Is this something that requires immediate medical attention? Or can it wait?

Balance between taken and spended calories need to be evaluated.

Weight loss is primary a matter of calories, so, first there is need for an evaluation of calories that you take and those you spend according to physical and mental activity.

This issue should be discussed and evaluated by a Dietician.

If the evaluation by the Dietician results in mismatch between taken and spended calories, other causes like thyroid function, kidney and liver disease, or other most serious conditions should be evaluated as possible causes of your weight loss.
