What Causes Venous Leakage?

Question: Hi I am currently taking cialis 5mg daily, I am not having the success i would like with it. As soon as I stop simulating and try to put my penis in my girlfriend my penis goes soft. I only get weak morning erections if I ever do get them.

I think I might have a venous leak, I do have a appointment on Friday my question is do injections such as caverject have better success rate then cialis/viagra. Or do vacum pumps have a better rate?

I have tried a penis ring as well when I am hard but as soon as I get up penis gets softer and softer.

Hi,
I feel that in your case, a venous leak is a significant possibility. I only wish to know if apart from taking cialis at 5 mg on a daily basis, were you also taking one 10 mg tablet on an on-demand basis, because I think you could possibly benefit from using it on a couple of occasions a week.
I have seen patients complaining that the vacuum erection devices do not give a natural feel to their erection, the glans feels cold. Intra & cavernosal self injection might be a better alternative.
I suggest that you get a serum total testosterone checked at around 8am.
You certainly need to get a penile Doppler scan done, and wishing that it will not have any major problems.

Yeah it's strange really I thought I had one in the firat place but the doctor was like it's really rare.

Are injections more successful then pills for venous leak? If I can get some sort of erection with cialis. Should injections do the trick?

Other then that should i be thinking about a penis implant? Or are there other solutions?

Hi,
Injections into the shaft of penis to achieve erections are more efficient in their objective. We have used a mixture called trimix and found it quite useful. But for a young man like you, I wonder how many months will you be able to continue to have the same, and also it can cause mild local irritation and plaque formation and curvature of penis.
Another alternative is an intra urethral medicine called alprostadil but I have never used it and therefore will not be able to comment on practical use.
I suggest you complete the penile Doppler examination and we will take it from there.

Thanks for your expert respone.

I am getting worried now because supposed to get married next year. My gf knows about my problem but she thinks it's in my head and will go away.

So assuming venous leak is confirmed and I have heard the surgery has litle success. What are my other options? Should I be considering a penis implant? Or do I have other options?

To some extent your girlfriend is right, and there is a possibility that it could all be in your head, but more often than not, such patients respond to cialis unless in your case, your dosage has not been optimized for your use.
This is one of the reasons why you should get a penile Doppler, and once the reports are available we will be in a better position to discuss matters. Penile implants are the last resort, though they are successful, as good ones can get to be ridiculously expensive.

I madturbate today and I was rock hard during it and it lasted for about 25 minutes. But if I stand up I will lose it, so not sure what to make from that.

I do have a appiontment tomorow with my urologist where I willl ask for testing for venous leak.

Penis implant is something I do not want to do.

I do belive this has been caused by masturbating prone With a soft penis. There is a site online that claims I just need to teach my penis to be hard. Not sure how much truth is in that.

We do quite commonly see patients who report that that the quality of erections and climax is better with masturbation rather than with intercourse. But that us not too good for a conjugal life, and therefore when such couples ask me for my advice, I suggest that the male partner stops masturbation as completely as possible, and even avoid reading or surfing for porn. This will help avoid desensitisation, and hopefully their intercoyrse will become a source of pleasure to both the partners.
I do not think you should worry too much right now regarding implants especially now that you are due for the penis Doppler scan anyway.

Thanks for the advice

Had appiontment today going to see a specialist next week or two who will carry out doopler test.

The urologists I saw today feels I should be able to find a solution without going all the way to a implant. As I can get a erection with simulation. He feels that the injections should definitely work.

I know injections are not ideal but doesnt look like I have much of a choice.

I will try the no masturbating and see how i get on with that.

Few people online talk about keegals would you recommend them?

Hi,
Kiegel's exercises are more useful in patients who in addition to poor erections also have premature ejaculation causing significant distress to both partners. I doubt if it will help in your case.
I also hope that you will respond to the injections alone, and will not need any implants, though I should also tell you that I am expecting reasonably good response in your case just by altering the medications dosage. We will however wait for the doppler test report.
