Question: hi doc,

I have had an episode of unprotected sex on 4th April.
I developed alight occasional burning pain and low back pain, along with frequent urge to urinate, as frequent as ten to fifteen minutes. also noticed sperm colour is thick yellowish.

I have had myself tested for urine routine, blood routine, and urine culture. blood routine showed mild infection , urine was clean. I also tested for gonorrhoea and HIV, it's negative as well.

I have panic anxiety disorder so may be when I think of it, and mostly I can't take my mind of it, the symptoms becomes more pronounced because of my anxiety levels
please advise

Brief Answer:
I would recommend an STD-panel, with emphasis on chlamydia sir

Detailed Answer:
Hello sir and welcome.

Thank you for writing to us.

I have gone through your query with diligence and would like you to know that I am here to help. As an infection was established through the blood routine, we should initiate antibiotic coverage for you sir.

Also, the urine culture also reported a clean chit?

But I would also like you to know that sexually transmitted diseases can also change the colour of the urine, and so I would recommend a check on chlamydia and gonorrhoea which are the 2 most common infections that can cause the sperm to turn yellowish.

As you have ruled out gonorrhoea, why don't you check for chlamydia; but at the same time it would be better if you go for an STD-panel (which is a little more elaborate and will provide us with a more clear picture on all STD's.

I hope you find my response helpful sir. Please feel free to write back to me for any further clarifications, I am always here to help.

Best wishes.

thank you sir. should I test for Chlamydia lgG, lgM, LGA? I would certainly have the full STD screening done, but was worried about the incubation period for HIV and hepatitis. so thought I would do one after a month and three months. would that be sufficient sir?

Brief Answer:
1 month should suffice sir, but go ahead with the HIV DUO test

Detailed Answer:
Hello once again sir.

The NAAT test can be conducted, or just visit a clinic and ask them for the best test to detect chlamydia and they will recommend it to you.

As for the STD-panel, a month should suffice; but specifically for HIV go ahead with the HIV DUO test which is 99.8% accurate after the first 28 days of contact.

I hope and pray we can find the root cause and help treat you at the earliest in order to bring you back to a clean chit of health. Please do not hesitate to write to me anytime sir, I am always here to help.

Best wishes.

thanks sir. it's only been 11 days since then contact, but I have done HIV duo tests, which turned out negative for now. keeping my fingers crossed and will wait for one month. I am really worried, and it's in my head that current symptoms point to definite HIV. could that be true? please give me a honest answer, I want to start coping up with that possibility :(

Brief Answer:
I honestly feel HIV is unlikely sir

Detailed Answer:
Hi sir.

Honestly speaking, I suspect an STD, but more on the lines of gonorrhoea or chlamydia, but not HIV. HIV-symptoms do not occur this early in a manner described by you sir.

A HIV DUO test conducted before 28 days is inaccurate, and so the test you conducted was of no use. Get the test conducted on or after the 28th day from contact sir. But I feel it is unlikely. :)

Best wishes.
