Question: Dear Dr, I am 31 year old male. I lead a very active life which includes runnng, walking , swimming, cycling for more than 6 days a week. dont smoke, drink very rarely and keep a good balanced diet. From Last year have been experiencing some uneasiness in the chest area and upper apdomen. Have gone through all cardiac tests, ECG (showed Sinus Tachycardia at 106 bpm , Otherwise normal ECG){{Have to mention that while doing the ECG the machine malfunctioned and they had to change wires all over me which left me anxious and stressed out) , Went for TMT ECG which came out normal and negative for Ischemia and normal BP and heartrate response, and the Cardiac Echo was also normal. Also went for Cardiac Enzyme tests in Blood which also came back normal. Can you please advice me if i am in normal cardiac health. ? I have attached my reports here Another point to note will be that , i am quite anxious at the hospital, which tends to shoot by heart rate high. Thanks XXXX

Brief Answer: please see below. Detailed Answer: Dear Sir 1. Your ECHO, TMT, Cardiac enzymes are within normal limits. Beside a marginally high heart rate, your ECG is also normal. You have excellent exercise capacity which itself speaks of excellent health and good prognosis too. 2. There are many causes of chest pain. Your physician should evaluate for a non-cardiac cause. Anginal pain classically occurs on exertion/ stress/exercise, occurs behind breastbone and is relieved by rest. A pain which occurs for less than a minute, can be pin pointed by finger, can be elicited by touch or pressure, occurs at rest but is not provoked by exercise, lasts for days together is not likely to be anginal. Thanks for your notings Dr Sukhvinder. Its very helpful. I do have some anxiety in hospitals , has been there since childhood. Never liked being in a hospital. But this time in the ECG room, the ECG wires malfunctioned, they were changing the wires of the machine over my full body, which kind of got me worked up a bit and i guess, the high heart rate. My questions are following 1. Do i need any further tests to be done to ensure cardiac health or do i have a good cardiac health already ? 2. Should i continue my exercise. I run close to 4 Km every day without fail. 3. How do i get rid of this chest and upper GI uneasiness. My pain is like an electric impulse for 1-2 seconds in the extreme left corner of the chest and also sometimes in the centre. Thanks for your advice. Regards XXXX

Brief Answer: please see details, Detailed Answer: Dear Sir 1. No, you need to under go any further testing, especially as your trouble/complaint does not seem to be related to heart historically. 2. You should continue your exercise regimen. 3. As I told you earlier a pain lasting for 1-2 seconds and electric impulse like in nature is not likely to be anginal. Do get in touch with your physician for examination and ascertaining a possible cause. A neurological or gastric origin may be responsible if they are frequent Sincerely Sukhvinder
