Question: my friend age 42 missed a period and then had a period first time she has ever had cramps in her life, lower back cramps, last night a big blood clot came out in addition two pieces of flesh came out
what could this be

Hello
Thanks for writing to us with your health concern.
At 42, it is common to have hormonal imbalance due to perimenopause.
THe commonest reason for this happening could be - endometrial hyperplasia.
In this condition, the lining of the uterus ( endometrium ) builds up, and then gets shed irregularly ( sometimes not getting shed for over a month and getting thickened - resulting in a missed period ).
Then when it does get shed off, it shows itself as heavy bleeding, clots, pieces of tissue being expelled and cramps.
She should get this diagnosis confirmed by a Gynecologist.
She would require an internal exam, a pelvic ultrasound scan and hormonal levels - FSH, LH, TSH.
Also, an endometrial biopsy if required.
A Pap smear should also be done.
There is nothing to worry as the condition is easily treated with cyclical progesterone tablets.
Obesity, diabetes, hypertension, smoking etc may aggravate this picture.
