Question: Hi Doctor,

I am having PCOD since last 2 + years and last year i was on medication Diane to regularise my cycles.
Since this year XXXXXXX i am not on med and having periods normallybut sometimes there are gaps between cycles which are long.
Like i had my last cycle in May 23rd and then on July 7th which lasted for 10 days 17th July.
But since yesterday nite i found one blood clot and even today also.
Earlier my gynae have asked to have Pause MF med and then should i start that?
We are going to plan for the baby from Sept so not sure why this happened.

Thanks for the query to hcm ,
As you are a diagnosed case of PCOD and have been off medication since 7 months now, unless you have been practicing healthy life style changes and diet control with exercises to keep your insulin resistance down, ovulation may not be hampered.
But PCOD can be prevented but not treated completely. So this spotting in the mid cycle can be a ovulation bleed which generally starts in mid cycle and lasts for 2 days with mild abdominal cramps ( mittelschmertz). This can be confirmed with an ovulation kit.
however if the test is negative then you may watch the spotting for at least 3 days and if it continues then you may start pause-MF tab which shall help stop the bleeding.

Thanks Doctor for your reply. So asuuming i need not to worry about this spotting right.
Also wanted to check as we are trying to plan for the baby from Sept any precautions needs to be taken for that so that it happens in one click.

Hello,
Well if you are planning in september then you should start yourself on TAB FOLIC ACID 5mg daily for next 3 months from now. This shall help in preventing any congenital anomalies in your foetus if you conceive.
There is no certainty for conception in one click and no trick to it. You just need to keep a watch on your ovulation period with the help of ovulation kits and practice timed intercourse during the fertile period.
Maintain healthy life style and dietary habits to keep insulin resistance low and try to keep your body weight in check well within the BMI limits of less than 25.

Hi,
Thanks for your prompt response. Ok and when should i start taking the tab, once a day and after what meal?
Also wanted to know how to stop this spotting also.regarding Body weignt i have put on weight due to PCOD and also any other medications to take.
Also so much of irregulaties in period how wil u undertand the ovulations in days,

Hello,
1. start folic acid 5mg daily from tomorrow half hour after breakfast and continue.

2. Observe spotting for few days and if it doesn't stop then you may take pause MF twice a day fro next 3 days to stop.

3. ovulation can be checked with help of ovulation kits from day 10 daily till a positive result which indicates that lh surge has occurred and ovulation shall tale place in next 36 hrs.
4. Weight loss is a must for decreasing insulin resistance and it is not supportive for ovulation at all.
