Brief Answer:
Few investigations/ soap water enema.

Detailed Answer:
Hi Sir, With regards.

Considering your age of 86 the possibility of fecal impaction giving rise to body's efforts to push-out the feces can give rise to pain, which is intermittent as you rightly noted, Sir.
In the result only mucus is passed out as it is produced by the body/ large intestine in excess to facilitate the expulsion of the fecoliths.

The recurrent nature suggest that this is an old problem, gets symptomatic on and off.

I would recommend the following in such patients:
Plain X-ray of the abdomen to see the level of fecoliths, area of impaction, possible air-fluid levels if the secondary intestinal obstruction is occurring.
Manual per rectal examination to palpate for any mass or rectal cancer pr fecoliths ( hard impacted stools)

I would first advise to go for a traditional soap-water enema. This works most of the times. Continue miralx in an appropriate dose to soften the stools, probiotics, Dulcolax suppositories and the other treatment as per the findings.
I would also ask for the CT safer side and colonoscopy too.

Manual evacuation is sometimes required as the overall strength in an old age is reduced as is the delayed passage of food to feces stage allowing lots of water to be absorbed to harden the stools. Due to reduced power of the muscles supporting the process of fecal removal is weaker and may thus need assistance.

Avoid all the foods and beverages which are known to you that cause constipation.
