What Causes Severe Pelvic Pain And Persistent Headache?

Question: I am having extreme pelvic pain on my lower left side. I had a total hysterectomy in 2009 so I know it is not related to anything gynecological. I believe that I recently had a UTI, which I am prone to getting because I am a Type I Diabetic. I have many autoimmune disorders, including Lupus, Fibro, and gastroparesis.
I am concerned about a kidney infection. The pain is intense and is worse both before and after urinating but tends to be at it's worst when I sit on the toilet for several minutes after urinating to see if I am done. I do have a frequency of urination as well. My blood sugars have been erratic, I have been getting headaches, and more importantly I have swelling in the area that is causing me pain. The pain has gotten worse over the last several hours, but the pain after urinating and sitting on the toilet for several minutes has been going on for about a week. I am concerned enough to go to the ER because the pain is getting severe. Could this possibly be a kidney infection or a bladder infection that has gotten worse? Or is it something else entirely? My headache has gotten worse tonight as well. I am in a lot of pain.

I have gone carefully through your query and understand your concerns. Based on the provided details it seems that you could have bladder infection (also called cystitis). It is a form of urinary tract infection (UTI) which is unlikely to extend to the kidneys. I would insist you to get a urine routine microscopy and culture sensitivity done. The results will help in excluding UTI as a possible cause of your pelvic pain. As of now drink plenty of water.

I would also need to ask you a couple of things. I need to ask you whether you are sexually active and whether you have stable partner(s). I also need to ask whether the pain seems to affect your hips and whether it radiates downwards to the limbs.

Hello, I have two children and have been with the same man for 12 years. He is definitely a stable partner, I am not concerned about an STD. We have not been sexually active lately as I've had some health concerns.
The pain does not radiate into my limbs, though I do feel it sometimes in my left hip, almost like a numb feeling.
I forgot to add earlier that I have been getting nauseous as well, however with my gastroparesis, I am unsure if it is due to that or the pain that I am experiencing.
Thank you for your help, I will get the tests done as soon as possible. If you can, let m e know if my follow up questions are of any further thoughts on a diagnosis.

Thanks for writing back.

Since you have been with one person and are not presently active, STDs can be kept out of consideration.

Since the pain does not radiate to the lower limbs, the chances of spondylosis being the cause are low. In such a case the first step would be to get an urine test done. The next step could be a pelvic ultrasound but the decision would depend on the urine report.

The gastroparesis and nausea are possibly unrelated.
