What Causes Right Abdominal Pain With Fever And Sore Throat?

Question: Hello my name is XXXX and im almost 25 years old, i ve been healthy my entire life(no meds no special cases) until 6 months when i started to have the exact same symptoms which are abdominal pain,fever and sore throat, i ve done a throat culture many times during the period and came back as normal flora, however my first sickness started in december then february then april and now may , all having the same symptoms (fever , abdominal pain that starts to the lower right and sore throat)i ve had a ct scan and turns out it s not appendicitis.however my fever goes up to 39.8 and reduced by itself without taking any medication and even if i do it doesnt help) my question is that could it be mediterranean fever??ps: each episode lasts fr abt 72 hrs

My Primary Impression-
Based on the facts of usg-and complaints-In My opinion- unless proved otherwise, You should not jump to FMF(Familial Mediterranean Fever),though its may be common in your region.

Most probable cause-in your case-could be-
could be-?TB abdomen with right abominal pains
Other causes of -Amoebic Colitis with typhlitis needs to be ruled out.
Mesenteric adenitis with Infectious Mononucleosis and FMF needs to be kept in mind in your case.

Follow up: Dr. Mukund Savaskar (5 hours later)
3 out of the 4 times I got sore throat first for 3 days before the abdominal Pain comes and the fever starts, this last time it started abdominal pain the fever and a day later a sore throat.
I don't have diarrhea or vomit with abdominal pain and I don't eat spicy food and when I'm in pain I feel nauseas
I'm not sure I can blame my diet on these recurring attacks because I ve been eating them all my life and it s hard to tell if they r the cause
I don't have a history of sore throat since childhood
I haven't tested my stools yet and I had mononucleosis 3 years ago
No history as far as my parent(mom) can tell but both my parents are middle eastern
We don't recall a family member suffering from cough/ fever with weight loss
I don't get joint pains per say but it does affect my join mildly( might be the knee or wrist) during the attack
My regular appetite is alright ( I eat 4 times a day)
I'm 65kg and 179cm
I workout it s been 5 years but when I get sick I loose weight and have to gain them back
I don't travel and most of my food is home cooked
The abdominal pain has no exact time but rarely in the morning as far as I can remember
I ve done the fmf test and I'll get the results on Monday and when I get my fever it sometimes comes with chills and sometimes not .

Hi Dear !! Thanks for your elucidate updates.

My Impression-and Plan of resolving your health issues-

Considering age and iP-location-In the given scenario and on the based on the updates and query facts,You seem to suffer from ?Mononucleosis with mesenteric adenitis with right abdominal pain accompanied with fever.

Possibility of -TB of Ileoceacal region with adenitis needs IGRA blood test and X-ray Chest-for detecting hidden active disease,as this could also give such complaints.
Amoebic Typhlitis with Colitis- needs to be ruled out by Stool Ova/Cyst test, which would rule out the cause of chills- which could be from Colitis and Typhlitis/or Ceacitis.

Prima facie- the history given does not lead to any polyserositis-like lungs/abdomen/joints etc. and hence FMF- seems remote possibility.Joint mains sometimes and not specific-could be from EB viral infection in your case.

But these are primary opinions and would need to be reviewed after getting above tests and Chest X-ray in your case and after FMF test report tomorrow.

Suggestion in current state-

Due to ethnic reasons, Will review your case after FMF- Familial Mediterranean fever-test report is coming tomorrow.
That would add more information in taking further decision.
