What Causes Renal Cortical Scarring While Having Diabetes?

Question: A 60 year old gentleman with diabetes on Insulin , but high levels of HBa1 c 7.5 and Renal cortical scarring on USG , Kindly opine on complications of diabetes ,chances renal involvement and impairment in near future .

Detailed Answer:

Hi,

your reports show a normal serum creatinine which is a marker of kidney disease . So you can rest assured nothing is going to happen to the kidneys very soon.

Diabetes does affect the kidneys in 50% patients.

1. look out for proteinuria.- you should do spot urine protein creatinine ratio test.

2. serum creatinine level every 3 month- now it is 0.87 as per report sent is absolutely normal

3. cortical kidney scars can be -- due to past urine infections or due to decrease in kidney blood supply

4. sugar control is very important- at present sugars are high


try to-

1. keep bp close to 130/80

2. keep pre meal sugars at 100-120 and post meal 2 hour sugers at 150-170

3. this is true for all 3 meals of the day.

4. identify if you have urine infection and treat it - do urine routine every 3 month when you do creatinine.

5. avoid pain killers and unwarranted ' Desi" medications

as far as 10 yr outlook of kidney is concerned it is good.

if you are under regular check-up nothing will happen
